I understood your health concerns.
I reviewed your data and In my opinion the repeated bleeding is due to the weak scar of the delicate prepucial penile frenulum, which gives way repeatedly and causes bleeding.
As frenulum of the prepuce of the penis is very sensitive structure,this needs immediate surgical correction.
I would advise you to consult ER Surgeon on this who would repair the penile frenulum for correcting its shortening due to fibrosis.

Current Treatment Plan-
Consult ER Surgeon who would treat it by ani-inflammatory and antibiotics.
After the healing of the wound in next 7 days time plan for the surgical correction of the shortened and fibrosed prepucial frenulum.

Plan the surgical correction at the earliest.

Hi,Dear,Sorry for the late reply.Was in one critical case.
Thanks for your updates and for the photograph.
The photograph has confirmed many facts for the cause of the repeated bleeding in your case.
The repeated frenular tears which you had in the past,have lead to the fibrosis in the frenular tissue leading to Frenulum breve or Short Frenulum,which is confirmed from the photograph,where in a white tissue over a pin head size area, and confirms that the fibrosis is restricted to very localised and very small area.
So as thought in my last reply, the frenular tissue needs to be released by plastic reparative surgery,if you are really concerned of the repeated bleeding and want to prevent any further attacks.
Decision for surgery would depend on your need and feel of discomfort caused from the Short Frenulum.
As you feel that you have normal sex life,its YOU who has to take a call and plan for the Repair of the Short Frenulum.
This advise for corrective surgery -is a preventive suggestion as the fibrosis is still limited to pinhead size area at this stage.And the results would be very good.
I would leave it to your need for it.
