Suggest Treatment For Testicle Pain

Question: 1. I had developed "occasional mild pain" on my left testicle (pain would be a 2 on a scale from 0 - lowest to 10 - highest), around 3 months back and continues till date. Now, this pain is also very occasional, around 2-3 times a day, there is a mild pain for about 5 minutes, and then, there is no pain. The pain feels like how it would be, when a pressure is applied on the testicle, and when the other side of the testicle is hardly resting on an object.
- A month back, I saw a very tiny pea-sized lump at the bottom outer side of my testicle, and that too seems inconsistent. Only at times (not all times), I would feel that there is a lump.
- I had a chance to see a doctor 2 weeks back, who surprisingly did not find any lump or swelling. Moreover, he ordered urinalysis tests, chlamydia, and gonorrhoea and everything was normal and the tests were negative (means, no diseases). He suspected some benign pain and prescribed anti-biotics (which I did not take, due to an issue with my insurance)
- However, I showed it to another doctor for a second opinion 2 days back, who did notice the small pea-sized lump, but he said that the lump doesn't seem to be 'on the testicle itself (but rather around the epididymis/vas differens). He felt it could be benign but did not rule out anything serious, and so, recommended me to do an ultrasound.
- I did an ultrasound today and the radiologist said "nothing big bad issue, you should survive, I don't have to urgently report this to your doctor, your doctor would get back to you".
I'm worried if this could this be cancer/pre-cancer? On a side note, when the pain is there, I do feel a mild burning sensation when I urinate (3-4 on a scale of 10 is the burning sensation). I did notice some red spots on the ultra-sound.

Thanks for your query,based on the facts that you have posted it appears that you have mild pain in Left testicle off and on since couple of months and routine blood tests and urine test are reported to be normal .The ultrasound scanning of the scrotum has not revealed any gross pathology .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Do you have painful ejaculation ?
2) Do you have burning urination since a long time ?
3) Was your urine culture done?

The mild pain that you have could be due to mild infection of spermatic cord (Funicullitis ).

Please consult qualified Urologist for clinical examination and get following basic
tests done to confirm the diagnosis.
1) Urine culture.
2) Ultrasound scanning of abdomen and pelvis
3) Color Doppler study of the scrotum.

In the mean while take broad spectrum antibiotic like Cefixime along with anti inflammatory drug like Diclofenac twice daily.

Using a scrotal support will help to get relief from pain .

Ensure to drink more water this will help to keep your urine dilute and relief from burning urination .

Further treatment will depend upon result of these tests and final diagnosis.
