Suggest Treatment For Symptoms Of Anal Fissure

Question: I have some rectal discomfort .It feels like I have a piece of stool that I can't get out . If I strain it has slight blood.when I wipe. This happens after main bowel movement. . I can get one piece out then it still feels like their is more. . This all started 3 years ago after I had Radiation for Prostate cancer. At first I had a lot of blood then it went away after a couple years. . now I have minor blood at times that goes away after awhile.' I also have low Platelets ITP and CLL I wras given Hydrocortisone 25mg supporsitories and a a Hydrocortisone cream 2.5% They work at times but the feeling comes back.I try not to strain .any sujestions? It can get very unconfortable..After I have main bowel movement ai might have to go again a 2 to 3 times with small pieces of stool. . When I wipe it is mushy and times takes a effort to clean it all off

Greetings of the day


Presence of constipation, discomfort and bleeding while defaecation point towards Fissure in anus as the most likely diagnosis. Radiation in the area of prostate could be predisposing factor. Exact diagnosis will however depend on good clinical examination.

I would suggest you the following which will be very much helpful to relieve your symptoms

1. Drink plenty of water everyday:. It will help by mixing with the fiber of diet and give bulk to the stools so that they are well formed and soft.

2. High Fiber diet: Vegetable salad( 2 servings a day), Fruits especially citrus fruits.

3. Avoid Diet causing constipation: Especially those food stuffs containing refined flour like cakes, bread, biscuits, cookies

4. Hot Seitz bath: Add a handful of common table salt in a tub of lukewarm water and immerse the bottom by sitting in it. Every such session should last for 15 to 20 minutes. Two such session a day, one in the morning and one before retiring to bed at night. It would also be benificial to repeat it after every bowel movement. It will help in healing of fissure, decrease pain and relieve internal sphincter spasm.

5. Local application of local anesthetic ointment like Xylocaine will help in relief of pain. Apply it 15- 20 minutes prior to every bowel movements and also before retiring to bed.

6. Nifedepine Ointment: Local application of it will relieve the spasm of internal sphincter and help accelerate fissure healing.


Almost all my patients have benefited from this by religiously following it and making it the part of their lifestyle especially water intake and high fiber diet.
