Suggest Treatment For Symptoms Of IBS

Question: Hello Doctor,

Please note I have following issues:

1.Have to sit in wash room for 30 minutes daily to empty the bowel. Once or twice a day.
2. I will have to strain a bit to have my poop.I feel the urge to go, but when i try to poop, i feel that the anus doesn't open up that easily.
3. Stool is normally soft, bulky or in small pieces. Sometimes It runs through (opening) fast.
4. Sometimes the stool is pencil thin and rarely foul-smelling.
5. Bloating & excess gas, sometimes difficult to pass gas even. And if the gas is expelled, it will be very loud. No belching.
6. Sometimes I have gurgling or rumbling sounds in my belly.
7. Some days of the week are good and some days are bad.
8. I have been going through this in varying shades for the last 10 years, but after taking isabgol and probitoics it has helped a lot. But some issues still remain as mentioned above. The improvement now is close to 70%

Sorry for delayed reply.
As per the history given by you it seems that you are suffering from IBS from many years.Irritable bowel syndrome is a very common disorder which affects the colon (large intestine). It causes constipation, diarrhea, gas, bloating, abdominal pain and cramping. Irritable bowel syndrome is usually long term and patient usually don't respond to medications for many years.
In some some cases, the opposite of the things mentioned above occur, this can be characterized by weak intestinal contractions, which slows food passage resulting to dry and hard stools.

Stress- Most people who have irritable bowel syndrome find that their symptoms worsen and are more frequent during periods of extreme stress. While stress can aggravate these signs, it doesn’t cause irritable bowel syndrome.

Take
Asafoetida-for Intestinal gas, upset stomach.

Food suggestions-
Eat a moderate amount of soluble fiber. It adds bulk to your stools. Good sources are whole wheat breads, oats, barley, brown rice, whole-grain pasta, the flesh of fruit (not the skin), and dried fruits.
Don't eat foods at opposite temperatures, such as ice-cold water and steaming hot soup, in the same meal.
Stay away from broccoli, onions, and cabbage. They cause gas, which can make you feel worse.
Eat smaller portions.
Drink water an hour before or after meals, not while you eat.
Keep taking probiotics preferably Vizylac twice daily.
Use only 1tsf of isabghul. Excessive fibers can cause severe bloating.

Dear Dr,

Thanks for your response.

Few more questions

1 Some days I feel my internal sphincter is not relaxing like usual while pooping. What do I do? Can I use nitroglycerine ointment?. If so how long should I apply them?.

2 Is there any benefit in biofeedback therapy or BOTOX injections in my case for relaxing my internal sphincter?

3 Some days I feel rectal pressure while sitting.
What is the reason and what is the solution?

4 Some days I produce lot of gas however it is odourless. It makes lot of noise while being expelled. It will be 1 to 2 hours after eating food. I do not know whether it is due to swallowed air or due to the food consumed?

5. My Vitamin B12 is 102pg/ml against minimum of 187 pg/ml and vitamin D is 8.7ng/ml against minimum of 10ng/ml. I plan to take tablets to improve these values.

Hello,

Answers are as follows-
1)Initially go for rectal examination of canal,Patency of sphincter would be checked by your surgeon.If its hyperactive then nitroglycerin can be used.You can apply it for unlimited time.
2)Botox can be used but initially medical management should be done.Most of the patients are relieved by nitroglycerin.

3)Initially i would suggest you to go for examination of sphincter by a surgeon secondly a MRI should be done to find out the exact cause of problem.

4) Take vitamin D tayo k 60k XXXXXXX once a week for 4 week then once a month for four months
Take B12 Myelamin 1500mcg once daily for 12 month

5)Bloating and acid can be caused by any kind of food. In most of the cases, it's fatty foods which cause these symptoms. Any food whether it's fat or protein or complex carbohydrate when can not be properly digested, releases large amount of gases like hydrogen, methane, carbon dioxide which causes burping.So try to eat food without too much fat.Take a probiotic regularly like econorm.
