Posted on Thu, 15 May 2014
Question: I'm a 52year old man,6 foot,195lbs with relatively good health. I have swelling neary scrotem where my inguinal canal is on both sides. My doctor has been treating me for epiditamitus for the last 5 months with antibiotics. I was tested for testicular cancer and had a vascular study and all was normal. I do not have insurance so I've paid for all of this out of pocket. I feel my doctor is not treating this as aggressive as he should, and I'm very, very concerned. Can you please give me some insight!

Brief Answer:
Possibility of bilateral inguinal hernia.

Detailed Answer:
Hi, XXXXX,
Thanks for posting query.
From your history it seems that you might be having some other problem than Epididimitis as after prolonged treatment with antibiotics there is no improvement.
First I doubt about having bilateral Inguinal hernia giving swelling on both sides of inguinal canal.
To clear doubt I request you to send detail about following points,
1, swelling increases while standing.walking or doing cough reflex or raising intra-abdominal pressure.
2, swelling subsides during sleeping or lying down position,
4, can you able to push bulging inside by pressing finger on bump,
5, any pain while swelling is there.
6, any gargling noise coming out from swelling.
After clarification it gives some clue regarding your problem.
There is likely of having bilateral Indirect uncomplicated inguinal hernia.
Consult surgeon and get examined.
With physical examination it becomes easy to diagnose your problem.
So if you send above mentioned details, I can throw more light about your problem.
Ok and take care.

Yes the swelling is more pronounced after working or long standing and reduces after sleep. I do have discomfort but nothing severe. I don't know if I can push it back in and i haven't heard any gargle noise. It all started as a small point of discomfort on the right side of penis by the scrotem.

Brief Answer:
Possibility of having inguinal hernia

Detailed Answer:
Hi, XXXXXX,
Thanks for posting the points I asked for clarification.
As the swelling is reduced while sleeping and increases during walking or standing it clearly indicates that you might be having inguinal hernia, direct or indirect.

Due to weak abdominal musculature or lax inguinal, canal some part of intestines or some fascia or fat stuff enters into lax inguinal canal giving rise to protrusion in inguinal canal giving rise hernia.
Do not try to press lump back into abdomen with fingers.

It seems that you might be having bilateral, reducible, uncomplicated direct or indirect inguinal hernia.

Avoid constipation.
Avoid cough reflex.
Avoid lifting weight.
Consult surgeon and get physically examined. If he makes a diagnosis of hernia, go for surgery.

I hope I answered your query.
Now if you do not have any further query or clarification, you can close the discussion and rate the answer.

I wish you good health.
