Brief Answer:
Few tests need to be done

Detailed Answer:
Hello sir,

After going through your medical query I would like to tell you that there are two important factors that could lead to swelling of the ankles.

1. Kidney dysfunction
2. Cardiac dysfunction

So to find out what is the cause in your case, I would like to suggest some basic tests for both the systems.

A. To rule out any kidney disease I would like to advise you to get blood tests done for kidney function, serum uric acid levels, ultrasound KUB.

B. For cardiac evaluation, I would like to suggest you take an EKG and ECHOCARDIOGRAPHY to see how the heart is performing and whether it's the reason of your ankle swelling or not.

And till that time, while lying down, keep your feet a little elevated with the help of pillows to avoid swelling. Consume a little less water than usual, also restrict extra salt intake (if any).

I hope this information will be helpful. Let me know if I can assist you further.

Kind regards
Dr. Bhanu Partap
