Brief Answer:
requires aspiration test, blood tests, MRI, consultation

Detailed Answer:
hi

Bilateral swelling of ankles without history of trauma or any history of fever,or other joints involvement needs thorough work up like blood test for arthritis profile, serum uric acid, serum proteins, creatinine, Na levels, K levels and check up by a physician and rheumatologist/orthopedics doctors.

In all probability there is joint effusion and aspiration cytology and biochemical test of aspirated joint fluid may be required.

Besides this, x rays and MRI of ankles for detailed view.

All this may be carried out step by step to find the cause and suitable remedy.
It is almost not an thyroid drug cause although you may speak to your thyroid specialist also
As of now, he may have to be on anti-inflammatory medicines like celeberex, serratiopeptidase and some anti-biotics and maybe prednisone if your doctors agree to give patient some relief.

best wishes
