I would like to know what is on my penis.

As an Urologist, i can fully understand and share your concern.

From the photograph attached, it looks like an inflammatory swelling.
You should get it examined by an Urologist urgently.

Inflammatory swellings respond quickly to treatment.
A course of antibiotics and anti-inflammatory medicines may be sufficient.

You should get some tests like :
1.blood routine, creatinine, sugar,LFT, VDRL, HIV and HbsAg.

You may send a copy of the reports here,for an expert opinion.
Maintain good penile hygiene.

Wish you well and a speedy recovery.

It can be caused by HIV?

If the test done recently is negative,it's unlikely to be HIV.

However, if you're having sex with multiple partners, or an infected partner, you should check the full STD panel of blood tests, in your own interest.
