Hi,
Thanks for posting the query on HCM.
After going through your query, you seem to be suffering from COPD with Rheumatoid arthritis. Currently you are on nebulised foracort for COPD. You also take amlong.
I would like to comment the following:
1. Do you have systemic hypertension (high BP)?
2. At present the following causes need to be ruled out - Cor pulmonale (Pulmonary hypertension secondary to COPD), CCF - ?secondary to high BP, Rheumatoid arthritis, Gout. Swelling due to medicine is less likely in your clinical scenario.
3. Investigations required are :BP measurement, ECG, 2D echo, chest Xray, RA factor, Serum uric acid.
4. Depending on your evaluation profile, the treatment will be required of the cause.
5. You thus need to get yourself clinically evaluated in detail by a Pulmonologist.

Her B.P is 140/80 and is consistently at this level.Her Echo and ECG indicate a good heart condition . She suffers from RA for nearly 3 decades and her condition is now fairly ok and rather painless.

1. If all the information and reports provided are of the last 25 days then , then please get the quantitative titres of RA factor and serum uric acid done along with chest xray. Flare up of RA activity needs to be ruled out.
2. If these are previous reports than repeat fresh reports may be required.
3. A close follow up with Rheumatologist and Pulmonologist will be required.
4. That her condition is painless is a good sign but needs to be evaluated.
