Brief Answer:
needs to see doctor

Detailed Answer:
Hello and thank you for your question.
This could be a blood clot or an infection called cellulitis. It is possible to form blood clots even while on blood thinners.
Please either get him to emergency or an urgent care for evaluation and treatment as soon as possible.
If it is an infection he will need antibiotics so that the infection does not spread.
Please let me know if you have further questions
