Posted on Mon, 28 Jul 2014
Question: Hello Dr.Kanodia,

Hope you are doing well.

Following your advise as well as medications provided by my doctor,my skins burns have come down to a large extent. Thank you for that. There are 2 questions which I have now:

1. I had a sun burn on the tip of my nose which has now left a mark. Attached is the image. I have applied momisoft and flutivate before however this mark doesn't seem to go. It looks weird considering it is at a very prominent region.

2. From the last 1 year there are some brown patches on the left side of my chest. These were not present earlier, my doctor has told me it was due to a fungal infection and it has now left a mark. I have attached images of the affected area.

Could you please help me in getting rid of both these marks please.

Thanks in advance.

Brief Answer:
Kojic acid with arbutin & Azelaic acid cream

Detailed Answer:
Hello young fellow,

Welcome again to the forum. I am good; tell me how you are doing.

Thanks for sending the details and the pictures so that I can guide you best regarding your condition.

After analyzing the pictures I can get that the condition is known as "Post inflammatory hyper-pigmentation" or PIH at both the places.

To tell you simply, the darkness or marks are due to the activity of melanocytes in the skin. These melanocytes are the cells which regularly produces color/pigment known as melanin in the skin. These are superficially present in the upper skin known as epidermis.

This is a tendency in us to get higher activity of these melanocytes after any minor to major irritation or injury .

The area over nose got injury or irritation due to sun burn while the area over the chest could had been irritated by fungus. The aforesaid areas being prone for injury to thesuperfical skin by various reasons got over production of melanin in the skin. This pigmentation is known as "Post inflammatory hyper-pigmentation" or PIH.

If I were your treating dermatologist then for your marks I would have prescribed a two kinds of cream (under dermatological supervision). The one for face containing Kojic acid with arbutin and mulberry extract in it. For chest I would have preferred for Azelaic acid cream.

These are one of the best and safest creams to be applied. Apply both these creams in a very thin layer at night time only so that these should not be visible to naked eye. Do not try to rub or massage, rather just touch the creams over affected area (especially over face). Apply at night time before sleeping.
It will take around 8-12 weeks for getting the marks lightened so have patience for the same.

Do not forget the part of sun protection and suggestions given previously. My all best wishes for your glowing skin.

I hope these informations will help you in getting rid of pigmentation marks.
I will be glad to reply any further queries.
