Suggest Treatment For Sore Throat And Fever

Question: Hello!
I eant to ask something about my sorethroat. 3 weeks before I had sore throat and a fever that eas 37. 8 and after that my fever vanished and remained only the sore throat. I went to the doctor and he told me ti use ibubrofen for 4 days. It was only one tonsilit swallowed. Ine week ago my 3 year old daughter got sick from the tonsilit with gigh fever. She went to the hospital and after 7 days she got better. Now from two days again my sore hurts but a litlle and I have no other symptoms and also I don't have fever. I saw white patches that I don't know what might be. Is it possible to have throat infection if I dont have fever? What do tou suggest me to do? I am affraif of using antibiotics because of some previous episodes of reaction. Meanwhile I am attaching a photo and I hope this does not make you feel uncomfort.

I read your question carefully and I saw the photo.

It will need 3 of these 4 symptoms to decide the indication of an antibiotic:
- White spots on the tonsils and swelling (you have this)
- Tender and swollen lymph nodes in the neck
- Absence of a cough
- Fever (this is a must and is over 38 degrees celsius in general).

So seems like you do not fulfill those 3 criteria for now so I would say to wait.
Take Ibuprofen as needed for a sore throat.
Do gargles with salt to disinfect and clean the tonsils- sometimes food gets stacked in the holes of the tonsils and looks like white spots. It will also help with a sore throat.

If fever or any of the other symptoms start, do let me know so I can suggest what antibiotic to take and also let me know in advance which was the antibiotics that gave you those allergic reactions previously.

Thank you for the response. I think that my lymph node in the neck is swallowed. It was since the first time that I had sore throat, it went away after i took ibrufen and now it appeared again. It is not big but it is there. I also dont have cough and it seems that fever doesn't tend to raise.
I had allergic reaction from ceporine, cefalosporine.
Can I be sure that using other kind of antibiotic I will be safe? Thank you again

It is normal that the lymph node will be there, because it needs at least 4-6 weeks to decrease fully in size, but as soon as it is not tender, then it is not acutely enlarged.

I don't think you need antibiotics if you do not have fever.

IF fever start being high then of course you can use other antibiotics.
In case of allergy, Azythromycin is an antibiotic that is recommended. And no, you don't need to worry of being allergic from it, because the allergies are very class specific. The full treatment that is recommended is: Azythromycin 250mg, 2 tablets the first day at once, then 1 tablet for 4 other days at the same hours, 5 days total, 6 tablets in total.

Thank you again. I want to ask one more question. One pharmacist told me that if you have no fever than this does mean that you body is not resistent and this is worst than having high fever. Is this true? Does this mean that i dont have strong immunity? She also told me that i have to start klamicid twice a day for 7 days.

You are very young and have no reason to have weak immunity.
A bacterial infection surely would give fever.

So if you don't have any fever, and if by now you have developed other symptoms like runny nose and cough, then it is just a common cold.

But if you have fever or even if after 3-5 days you are not getting better, I would suggest to take an antibiotic.

Klamicid(Clarithromycin) is an antibiotic and I would not recommend it unless it is recommended (see above).

To be absolutely sure, you can consider doing a rapid strep test (strep culture) who would tell for sure if it needs or not antibiotics.

Dear Doctor, thank you again. i star using Azytrimichin and I feel better. Meanwhile I made the blood analysis and as far as I read, I saw that I was really anemic. Please can you watch them and tell me if the results of Fe are problematic? And can this be regulated with food balance.Thank you again!

Thank you for the follow-up question.

I reviewed your lab results.
You clearly have Iron deficiency anemia.
This cannot be balanced by diet alone, as there is no food who has enough Iron as so replenish the empty deposits of Iron. You need to take it twice/day, before food if possible but take it with food itlf it gives you many side effects, and for at least 3 months. Check blood test again in one month after using Iron to see how is the increasing of Hemoglobin going.

Ok thank you. How much miligre daily should I take?
Another last thing please.
Is it normal that still I do have sore throat, and specially associated with pain in the back of the neck and earache? Should I contact any doctor. Today is the 3-rd day of using antibiotic and still I dont see any good results. Thank you

Hello,

You need to take Retafer(Ferrous sulfate) 100mg or Heferol 350mg, twice/day, before food and with one tablet of Vitamine C (depending on your location, you may need a prescription by your local physician to get Iron supplementation from the pharmacy).

You may start having black stools and metallic taste in the mouth- these are usual side effects of Iron supplementation, don't worry.

If you cannot tolerate that, then use just one tablet or Iron before food, and if that is still difficult, use it after food.

The antibiotic should start working after 24 hours. If it is not working, that means it is not a bacterial infection, so you didn't need the antibiotic in such scenario.
