Suggest Treatment For Soft Tissue Injury On Tibia

Question: my brother just got hit in the tibia with a piece of wood with a nail. he had a tetnus a year ago. it swelled up immediately and lots of pain. he is on warfarin 5 mg per day for blood clots. when i sat him down and elevated with ice, 20 on/20 off the swelling went down so i thought he was ok, the moment he stood the pooling of blood came right back and the bump is big again. he is 51 and was hospitalized for blood clots about a year ago. I am fairly certain it is a bone bruise but my concern is if he can wait to see his doctor tuesday morning or if i need to get him to hospital now. he does not have insurance so waiting is optimal but i do not care if it means something has to be done right now.

From description it seems to be soft tissue injury or bone bruise casing the symptoms. As fracture is not likely, you can wait to see his doctor tuesday morning. Meanwhile continue doing icing and elevate the limb. Avoid putting weight and rest well. He can take anti-inflammatory drug like ibuprofen to control pain and inflammation.

you are not concerned that this will break off (the hematoma?)
also, please explain why when icing and elevated the swelling goes away almost completely but when he stands it appears quite quickly.

As haematoma is not organized, on elevating the limb swelling goes away almost completely and appears quite quickly on standing.
Consult your doctor.Your brother might need dose adjustment of anticoagulant drug warfarin.
