Brief Answer:
Changes as per the age and exposure to sweating as discussed

Detailed Answer:
Hi.
Thanks for your query.
Read your history and seen the pictures The skin changes of the scrotum go with the age related changes.
Excessive sweating, too tight underwear, hot atmosphere cause the seborrhoeic dermatitis in the scrotal skin.
The treatment is as follows:
Clean the scrotum and penis with tap water 2 to 4 times a day.
Allow it to dry up or use hair dryer.
Use loose underwear and pants.
Avoid hot and humid atmosphere.
You may apply Liquid Povidone Iodine locally once a day after cleaning and drying the skin to keep the area sterile.
Take multivitamins as they help for good skin integrity.
Do not over clean or rub or itch the skin as it allows trivial injury allowing infections to set in.
There are moles on the skin, watch if any changes in color, size, shape, border, surface or itching- consult a Dermatologist if such changes are seen in these black spots. They are moles and not to worry about if there are no changes over time.

The little pimples on the skin are normal for your age, hence stop worrying about them.
Applies the same principles of care as mentioned above.
