Suggest Treatment For Sinus Tachycardia

default
Posted on Mon, 6 Apr 2015
Question: Hello Doctors,

I have been diagnosed with Sinus Tachycardia last year but with no other findings, no ST elevation or ischemia, I was overweight then, I rediced the weight and now I am in healthy BMI weight ratio, after all this thing I decided to recheck with ecg, it came out as borderline ecg 800 sinus sythm normal, Remarks were : Within normal limits, BPM was 96, then I further decided to go ahead with echo to rule out and underlyng cause, the echo was fantastic with ejection fraction as 60+ and everything normal.

At home when I chech my BP and heart rate its always less then 118 the systolic and less than 78 diastolic without any medication. But there are occassional bouts of tachycardia especially when I am tensed or when I dont get good sleep, also bit anxious all the time.

At doctors office sometime the BP is 140 but at home it never dares to crosses 118 the systolic and never crosses 80 diastolic.

The question is do I need to still worry, even though ECG and Echo says ok ?

Dont have any symptoms or issues, but just that normal bouts of fast heart rates is what worrying me I am all the time

Brief Answer:
Counselling helps / Investigate further, if symptomatic

Detailed Answer:
Hi friend
Welcome to Health Care Magic

The description is highly suggestive of Anxiety
Anxiety causes tachycardia / Tachycardia causes anxiety – it is a vicious cycle
Your doctor’s prescription of atenolol is appropriate - it is a Beta-blocker that helps both anxiety and tachycardia. You will benefit from it or any other from the same class. The dose is small and very reasonable. This prevents excessive raise of heart rate and pressure from anxiety or work

Your Blood pressure high only at the doctor’s office is called White Coat Hypertension. It, again, is from Anxiety

You will benefit from Psychological assessment and assistance

A normal ECHO is reassuring indeed / it has ruled out valve, wall problems; shows good function…
Get investigated for Thyroid – over activity can be a cause
The normal range of heart beats is from 60 to 100 / you are still in range
If you are worried, a TMT (Treadmill Exercise-Stress ECG) can exclude ischemic heart disease. More to exclude rather than entertain the problem – to help one to relax

If there are no clues and if the problem is still bothering to the extent of interfering with life style, we may opt for more investigations of increasing complexity – Electro Physiological Studies (EPS) > it resembles angio – a catheter is put inside the heart / electrical activity recorded / stimulation and suppression tests are carried out / suitable medicine tested and so on. There are other advanced treatment modalities - like ablation (thermal or radio-frequency), which could be done during EPS. Though the test is the gold standard, it is INVASIVE and hence is not generally done unless there are compelling indications. This super-speciality expert is called ELECTRO-PHYSIOLOGIST.
I feel, you do NOT need this. The treating doctor may suggest them depending on need, based on his assessment of the situation.

Relax / Enjoy life / Follow up with your doctor, for any problem

Good luck
God bless you

Hello Doctor,

Thanks so much for the response its reassuring for me.

I dont have any symptoms, I can do brisk walk, Jog as well without any issues, ofcourse I am not an athlete, I had my TMT stress test 6 months ago, it has ruled out ischemia as well with no ST elevation or anything.

I forgot to mention that at Home my heart rate is always under control ranging 65 BPM to 85 BPM at XXXXXXX

Its just that I had concerns with high heart rate sometime, ususally happens when I dont seep well or when tensed or When I think too much.

During mild fever and cold my heart rate was 115 so I was worried at that time too, but now that the fever is gone the heart rate is ranging 65-85 depending on what I am thinking or doing.

I take no oil and less salt too.

The reason to do such test was I face difficulty in sleeping I further investigated and found that due to the nature of my work 24/7 the sleep pattern may have been disturbed, also my back shoulder blade was paining, further investigation found that its musculoskeletal pain since the symptoms do not match that of cardio, since whenever I do any activities there are no pain and the pain is stable gets worse when I fidgit with that pain :) but no pain during brisk walk, running or any exercise, I can climb 7 floor stair case without any issues that to non stop and even good to go for 2 more floors to run out of breath.

What do you recommend in that case should I be still worried, since a response from a doctor is always reassuring.

Regardless of me taking atenolol or not my heart rate is always normal at home, thus I plan not to take it unless I really find my heart rate shooting above 100 all the time.

Appreciate your response in advance doctor thanks a ton for the earlier reply :)

Brief Answer:
You have a good heart indeeda

Detailed Answer:
Hi

There is obvious reason like fever or anxiety for the heart rate raise. It is normal and expected response. No biological value is constant – failure to vary is a sign of disease!

Good that the TMT is negative for ischemia. You have excellent effort tolerance – very few can climb seven flights of stairs… Your heart is good indeed. Your Life style too sounds good / continue exercise – at least half an hour a day, for a minimum of 5 days in a week

Situations remain the same – it is our outlook that decides the reaction. Aptitude dictates the altitude. Relaxation exercises, Bio-feedback techniques, Yoga etc. are a few helpful guides to reduce the tension and relax. Focussed work helps. Taking up a hobby helps. Rapport with family and friends helps. Learn to ignore the symptoms and enjoy life…

But Doctor it really becomes difficult for me to analyse why my heart rate is so high at doctors office it make me rethink, At home the heart rate is 65 - 85 but at doctors office its 90+ at time it reaches 100+.

My BP is always Hovering around 105 to 119 systolic and 68 to 82 diastolic at home, but at doctors office the figure is spoilt its 140 the doctor says does that mean I need to consume BP tablets, infact my Mom and my brother strive very hard to get to the figure that I have that is 105 to 119, yet how can I have hypertension.

Also doctor give me good tips on how to achieve a good sustainable sleep, My nature of work is 24/7 where I have to work in Day shift for weeks and night shift for weeks, any tips to keep my cardio in top notch condition will be highly appreciated, but remember I am not an Athlete :) but as said earlier I can do brisk walk for an hour and climb 7 floor through staircase without no pain or major difficulties.

Thanks a ton doctor for your earlier answers, couldn't stop thanking you as the doctors outside prescribe medicines without even realizing the long term side effect :)

Brief Answer:
Counsellig helps

Detailed Answer:
Hi

That is because of subconscious fear of facing the doctor/ and worrying about doctors’ examination response. Why should we worry about the future, which is unknown / Enjoy the present
You need to shed fear and negative thoughts and nurture positive thoughts /that is where the counsellors, family, friends, and religion play a role. Medicines are necessary evils - they do have side effects, but their benefits must outweigh the disadvantages, in the given individual

If the job is the reason, look for an alternative –that can suit your timings. Even the income is less, it gives peace of mind and body

Thanks doctor for the earlier responses, today I had a blocked nose I generally suffer from sinus, in order to do my usual brisk walk I opened by blocked nose using otrivin, opening the nose caused sensitivity in throat and chest and while doing brisk walk my throat became dry and spasm occured in my windpipe on the right side chest, which went away after I rested. Tomorrow I will again jog this time without otrivin to observe things.

What do you think with your expertise, I was asthmatic before but during exercise I was not breathless or anything serious just that spasm on right side chest felt with cold air after taking otrivin to open polyps.

I think its the opening of polyps in nasal that has done the trick here.

Thanks doctor :)

Brief Answer:
Allergic rhinitis and bronchitis?

Detailed Answer:
Hi

The nose communicates with the throat at the back
Nose drops inevitably trickle down to throat / probably a little irritation to the wind pipe and its right branch.
Use the drops, with the head tilted to the side / use a lower concentration (0.5% - paediatric). Asthma could be silent – get examined by a Pulmonologist (lung specialist) / PFT (lung Function Test) may show. A CT scan of the para nasal sinuses can show the cause – Polyp, infection etc. It may be allergic…
