Suggest Treatment For Shortened Cervix During Pregnancy

Question: Hi, I'm 26 and am currently in my 28w 5d of pregnancy. My cervix has shortened upto 2.2 and a Calzaghe was placed at 22w. In the scan I had around 2 weeks back, the stitch was in situ b but intermittent funneling was detected. Since then my bed rest had become more strict. I'm going for a repeat scan tomorrow and hope everything is good. the bed rest did help my cervical length to get back to 2.5.but the baby's growth and to show a lag of around 10 days. In particular the AC parameter showed a Lag of 2 weeks and was in the 5th centile. The other parameters were close to perfect. Is there anything worrisome? My doc is worried about the funneling and the growth being less. She said to wait and watch and we'll know in tomorrow's scan. I wanted another opinion on this.

Hi XXXXXXX I have gone through your question and understand your concerns. Short cervical length and intermittent funnelling is worrisome. You need to be on bed rest and avoid heavy work to prevent preterm labour and delivery.
Abdominal circumference of fetus with a lag of 2 weeks and at 5 th centile is suggestive of a intrauterine growth restriction. It can be due to nutrition deficiency, fetal infection, decreased blood supply to the fetus or maternal hypertension, diabetes or any chronic medical illness.
I would suggest you to get a Doppler ultrasound to assess the fetal blood supply, if normal then you should get your blood sugar levels and blood pressure checked. You should follow a high protein diet consisting of milk and milk products, pulses, eggs and non vegetarian items, if you can .

Hi doctor XXXXXXX

The doppler parameters were a problem at 20 weeks. After that I was started on ecosprin and at 26 weeks the parameters were normal. GTT results also seem normal and so is the BP. Diet also is protein and fibre rich. We aren't able to understand the cause of this problem.

Hi doctor,
The following is my daily diet intake:
-5 almond soaked overnight + half glass milk (with 2 spoons of protein x) before breakfast.
-3 rotis with ghee spread + 1 bowl vegetable + 1 glass milk with half spoon sugar for breakfast.
-half an apple, half a Mausambi, one banana, few pieces of musk melon or a small bowl of pomogranate after 1.5 hr.
-1 boiled egg after 1 hr.
- brown rice 2 bowls, 1 bowl Dal(all Dals on rotation daily), 1 bowl vegetable, and half plate salad (Cucumber, tomato, radish, beetroot)
-Fruits as mentioned above after 2 hrs.
-1 go milk with 2 spoons protein X and biscuits.
- 3 rotis, 1 bowl dal and 1 bowl vegetable,half plate salad.

Some days I take curd in the afternoon and buttermilk in the nights. I also have carrot halwa sometimes.

Hi XXXXXXX your daily calorie intake is around 2000 kcal/ day, but itcontains more of carbohydrates rather than proteins. I would suggest you to replace fruits at one time by sprouts. Have at least one litre of milk daily, either plain or as curd, paneer or chena. Try to have peanuts and roasted grams as snacks rather than biscuits.
