Suggest Treatment For Severe Vertigo In A Person After Suffering A Brain Stroke

Question: In year 2010,my mother has suffered brain stroke and she has been prescribed the following medications:
1) Morning:Volibo 0.2M,Seloken XL50,Nicardia R20 and Neurobion
2) Afternoon:Ecosprin 75
3) At Night: NicardiaR20,Atorva 5mg and Neurobion
On 23rd March,she suffered from severe vertigo along with vomitings.She was admitted in Hospital and MRI,2D-Echo were done.The doctors said that MRI shows no acute infarct so its a case of simple benign positional vertigo and she is on Vertin16mg and Cinzan25mg 3 times a day.
However,since her BP was well under control in the hospital,doctors advised to stop Seloken XL50 and NicardiaR20.From last 7 days,she is taking only 1 NicardiaR20 instead of 3 hypertension medicines.I have read thru net that beta blockers cant be stopped suddenly.Can you please advise is it ok or not?
Her current BP range is from 110/70 to 135/80 in last 4 days.He pulse is from 75 to 89.I think this has increased as it used be around 67 to 72.Pls suggest asap.

Your Mother had suffered from Severe Vertigo along with vomiting. This can be due to
1. Reduced Blood Supply to Brain
2. Benign Positional Vertigo

Now
Let us look at the first cause 1. Reduced Blood Supply to Brain

Reduced Blood Supply to Brain can occur due to
(a) Obstruction in Blood Vessels
(b) Low Blood Pressure

For Example,
When you open the Tap
The water has to come at full flow
If the water is not coming at full flow, then, it can be due to either low pressure in the Tank or blockage in the pipe

Same Way
The Vertigo and Vomiting can be due to Block in Arteries (Stroke) or just low Blood Pressure or Bening Positional Vertigo

You have mentioned that her BP was 110/70
THis BP can cause Low Blood Flow

So we need to increase the BP
So We need to Decrease the Dose of Anti Hypertensives (Drugs for BP)

Sir,thanks for replying.My queries are:
1) On the day of episode,her BP was high ie 170/80 so i dont think it happened due to low BP and since her MRI also has not shown any actute infarct so it could be related to Positional Vertigo.However,i am afraid of stopping beta blocker.
Please do let me know what should be BP and heart rate which would be considered higher to contact doctor for revising her dose.
Are there any other symptoms to be watched for due to sudden stopping of Seloken XL50

I have checked the Blood Pressure only during Vertigo episode n it was high and also doctor checked at ER.
Also,2day she suffered from palpitations..heart rate was around 125 to 140.We went to Emergency again.
The ecg was ok.Doctor has restarted Seloken Xl50 again.R der any side effects for restarting seloken xl50 again or its okay now.She has not taken Seloken for last 7 days.

Madam

The usual sequence of events is

Low Blood Pressure --> Low Blood Flow --> Ischemia of Brain Stem --> Vertigo + High BP + High Pulse Rate

BP would be always higher after a TIA. Hence a High BP During the vertigo or after that does not rule out the possibility of a low BP having caused this

This means that your mother needs close observation

She need the right dose
The dose should not be too low to cause Increase BP and increased heart rate
The dose should not be too high to cause low BP

The treating doctors would be able to decide at the correct dose by close observation and monitoring

If Pulse Rate crosses 90, you need to contact the doctor and dose has to be increased
If BP falls below 120 Systolic or 80 Diastolic, you need to contact the doctor and dose has to be reduced
