Suggest Treatment For Severe Tooth Pain While On Amoxicillin

Question: Hi: I had a "short" crown with a sensitive upper front right incisor inside break off flush at bottom of gum line – sorry for layman's talk. Honestly, this tooth has been slightly sensitive to touch for about six months. My great dentist who is out of town doing doctors without borders has two other dentists working for him. One decided to do a root canal. After the root canal, the tooth is not sensitive to hot, cold, direct air, but when I tap bottom, it radiates sharp pain. Again, the tooth is broken off and remains an open space. I've been on Amoxicillin for two days. I've been sore at very top of gum (septicaine shots?), but also have a hard bump next to my nostril. It's not sore, but assume it's related to this damage, or the root canal, or shots. I see my regular dentist Monday. If it's not sensitive to hot, cold, direct air and has had a root canal and I'm on Amoxicillin, do your think it's something structural? Maybe lose the tooth? Thanks so much for considering. xxxxxx

Thanks for your query. I have gone through your query and the image.

As far as the pain and swelling is concerned, it can be because of the peri-apical abscess or peri-apical cyst (infection in the root tip) or improper root canal treatment. Nothing to panic, you need to get a Radiograph( X ray) done to confirm the diagnosis.

Once the diagnosis is confirmed, we can treat accordingly. Meanwhile you can take a course of antibiotics like METRONIDAZOLE 400mg twice daily for 3 days along with AMOXICILLIN 500 mg 3 times daily for 3 days. These drugs are prescription based medicines so you need to consult your oral physician and take the prescription and you can start with these antibiotics.

Hi Dr. Kumar: Thanks so much. So the hard bump up in upper gum, next to nostril, is part of the issue – abscess or cyst? I'm already on Amoxicillin – two days. Back to dentist Monday, go from there. Thanks again..

The hard swelling can be secondary to the tooth infection. You can take the antibiotics which I advised and get the radiograph like intra-oral peri-apical radiograph done for that tooth and upload the images, I will tell you the exact diagnosis and treatment plan. Only amoxicillin will not reduce the infection, so you need to take both the antibiotics.
