Suggest Treatment For Severe Nausea Caused By Acid Reflux

Question: My son (17 yo 5'9 150lbs) has had problems with nausea for months. As soon as he begins to eat (within 2 bites or so) he becomes nauseated and often cannot continue to eat. Sometimes he can eat more but then has stabbing pain in his stomach afterwards. The nausea lasts about 10- 20 mins or so then he can eat a little more again. The stabbing pain lasts only 10 minutes or so. Nausea can follow the pains afterwards but not overwhelming nausea.
The first couple months, he lost weight - about 5 pounds. . My son is extremely athletic, very active in highly competitive sports and has absolutely no spare weigh to lose.
His doctor prescribed two different medications for acid reflux but neither worked. He had blood tests done. Nothing abnormal. He has an ultrasound that showed nothing, a gastroscopy that showed one lesion in his stomach that they biopsied. They also biopsied another part of his stomach to test for I-pilori. Both biopsies clear. Last week he had a barium swallow (to be timed intervals at 5 different stages) but was having a very difficult time getting the barium down in good time due to the nausea. The radiologist called it short after 3 swallows and images. The X-rays showed nothing abnormal. His bowel movements and stools are relatively normal sometimes soft but infrequent diarrhea. Blood tests redone showed nothing. Some days better than others for his ability to eat but he either becomes nauseated OR is able to eat larger quantity but has stabbing pain in his stomach afterwards. He can drink liquids but limited to about 2-3 cups before feeling nauseated. Now we have to wait a few more weeks for an appointment to find out what to do next. Then Wait some more. I wish we had a paid health care system so I could get my son the help he needs now. It has been 6 months of this. Make appointment. Wait. Have test. Wait. Get results. Wait. Make new appointment. Wait. Etc etc etc. Help?

I have gone through the medical history of your son and can say as following:

1. Can you please provide the reports of all the tests he has done so far? I need to have a look and analyze them by myself first especially when it comes to the lesion they noted in his stomach? Thank you!

2. As he has been nauseated and pain in the stomach immediately after eating that does not allow him to eat, apart stomach problems, that could be due to:

- gallbladder issues (sludge or stones) or other liver issues: an abdominal ultrasound is advised to rule it out
- appendicitis especially chronic appendicitis is also to consider - ct-scan of the abdomen is required for this (can send the barium test report to me to analyze it further and if they ruled out appendicitis)
- urinary tract infections (UTI), sometimes, they do not give urinary problems but only GI problems as in the case of your son especially if he is not circumcised. Urine analysis is recommended together with liver/kidney function blood test.

If I was his caring doctor would rule out gallbladder/liver/appendicitis/UTI as mentioned above. Can discuss it with his caring doctor about these my possible causes to rule them out.

Thank you. I cannot share any lab or test results at this time, as I do not have them. I will be trying to see family physician who ordered the ultrasound and blood tests or surgeon who performed the endoscopy/biopses and who ordered the barium swallow this week. He has not had ct scan. Doctors have not asked for urine sample. I will ask for this. My son is circumcised. Will ask about liver/kidney function blood test - i know two blood samples and tests done. Tests for: Glucose, TSH, Creatine, Sodium, Potassium, Chloride, ALT, Alk phosphatase, Bilirubin, CBC. I was told that there was nothing abnormal with these blood tests.

Thank you for providing further information.

I was reviewing the results of tests your son did, and noted that :
- kidney function were ok (creatinine ok, but no data for BUN)
- liver function was also ok (bilirubin, alkaline phosphatase, ALT, but nothing for AST)
- electrolytes were ok
- CBC was ok
- thyroid function was also ok (TSH)

I still think of UTI and appendicitis or inflammatory bowel disease. So, can discuss with his doctor to check them out.
