Question: Too long a story to write in detail. Suffice it to say I need a simple answer to the question can a definitive sinus infection (or polyp/related mass (not sure if it's fluid filled or not) in my ethmoid sinus (self diagnosed and missed in the ER by the radiologist and er doc today on my thin slice multi-sequence MRI (BUT EXTREMLY CLEAR TO ME on my Axial views) trigger cluster headaches? I have had typical Cluster headache symptoms for weeks (and had a cycle a few years ago). 02 helps the nighttime awakenings if I wake fast enough , predinosone helped for only a day or two, opiods and gabapentin are of no help. ER Doc consulted with a neuro doc in the hospital today and prescribed carbamazine for trigeminal neuralgia. I'll go get the infection treated tomorrow with some antibiotics and schedule a vist to an ENT, now that I know I have an infection, but I need to know before some other doc tries to tell my I have trigeminal neuralgia (which I unconditionally do not have), if sinus infections can in any way trigger cluster headache cycles? So, I can get those treated correctly after a get relief from the sinus infection.

Brief Answer:
Sinusitis related headaches differ from cluster headaches.

Detailed Answer:
Hello and thanks for using HCM.

I have read your question and understand your concerns.

Obviously there is a lesion that is filling your right maxillary sinus, but just one image is not sufficient to predict the nature.

Cluster headaches are caused by imbalances of histamine and serotonin in brain and have a hereditary component.

Trigeminal neuralgia also is a separated diagnosis not related to sinusitis.

So, the answer to your question is no, cluster headaches are not related to sinusitis.

Anyway, that sinus problem found by MRI should be evaluated further and consulting an ENT Doctor is the right way.

Hope you found the answer helpful.

Let me know if I can assist you further.

Greetings.

Ok Doc, I'll get the sinus issue resolved and continue to pursue a neuro doc for a better diagnosis and treatment for what I believe are cluster headaches. I was just hoping (more like praying) there could be some kind of link so that when the sinus infection resolves, the nerve pains might too. Thanks for the help.

Welcome back.

Glad I helped in some way.

Consulting a Neurologist and, maybe a second opinion should give the correct diagnosis.

Best regards.
