Suggest Treatment For Severe Hair Loss

Question: Hello,
I (Male, 31 yr.) have been suffering from hair loss since almost 2-3. Initially it got unnoticed. But since last one year it became prominent. It’s a kind of diffused alopecia whereby hair density became very poor in the crown and mid-scalp area. Also there are two adjacent small patches in the frontal area (almost a-1.5 sq. inch) having no hair at all. I have stated consulting dermatologist since last two months and continuing his prescription as: Finasteride 1 mg (once daily), Fluconazole 400 mg (once in a week) and applying Renocia Lotion onto scalp once daily. He also provided some solution and shampoo having coal tar extract to use sos basis to prevent dandruff. Again a mometasone furoate cream (Elocon) was given to apply in the small patches having no hair. Even after strictly following the prescription since then, may be the rate of hair fall has reduce a bit (not quite sure) but there is not any visible improvement at all. I have also attached two photographs of my scalp almost three months ago (before treatment started).
Kindly also note that from my childhood I had little body hair (common feature in my paternal side) and having less developed mustache and beard. But in last 2-3 years, all the remaining body hairs have been completely vanished (whatever hair remains in mustache and beard has also reduced in no.). Now even, the hairs in my eyebrows and armpits are almost at the stage before vanishing. Though my concern is my scalp hair, just mentioned this to let you know if it has any significance over scalp hair at all. I fear that if situation become little worse I would be completely bald very shortly. Kindly prescribe me whether I should continue this treatment or any other step is required before its too late.

Dear Doctor,
I have again mailed the photographs and uploaded also. Pls. let me know whether they are visible or not. The photographs are three months older (before start of treatment). From my childhood I typically had little body hair (almost common in my paternal side). But whatever body hair I had, those had been entirely vanished in last 3-4 years. Now even the hairs in eyebrows and armpit are being rapidly lost and may be extinct in next few years.
But my main concern is scalp hair, which has diffused alopecia over large area of crown and mid-scalp. The density of hair is really low. And also a few minor patches (with no hair) are there. Kindly let me know, whether my treatment is moving towards write direction or not. Is that I have any hormonal or internal organ related problem? Pls. note that my doctor has not prescribed me Minoxidil and instead I’m using Renocia lotion.
I don’t require any consolation, so pls. let me know whether my case is curable through medication?

Hi. I have reviewed the Images.
You seem to have both andogenetic alopecia, which is familial and affecting your scalp, as well as Alopecia areata, which is affecting eyebrows, axilla and other body regions.
If I was the treating doctor I would suggest you to continue with finasteride and renocia, as before. I would also treat you for alopecia areata with pulse oral corticosteroids i.e betnesol forte 5mg, on 2 consecutive days for a week.

Dear Doctor,
Thanks for your advice. Pls. let me confirm additional dose of Betnesol 5 mg for two days, thus total 10 mg has to be consumed right? Now would that be enough to tackle my problem what you've mentioned or some more would be required? Pls. let me know after how many months I should again consult you to review my progress.

Dear Doctor, do you really believe that I would be able to re-grow the my hairs which already have been lost or it's too late? I heard that alopecia areata is incurable.

Oral XXXXXXX Pulse is a good choice for widespread alopecia areata

Hi.

I a talking about, Oral XXXXXXX pulse (OMP) i.e 5 mg of betnesol forte, on two consecutive days a week i.e saturday and sunday. It is a well established therapy and is choice of treatment for wideapread alopecia areata and may last for a total of 4-6 months for a good response. Response may vary from individual to individual. Moreover, there is a chance that you may lose hair once therapy is discontinued, but to what extent, it remains to be seen. Mind you- this therapy is to be used strictly and only under supervision of a doctor. Therefore I suggest that you re-visit your doctor and talk to him/ her for the needful.
