Brief Answer:
I suggest a hair transplant along with medical treatment

Detailed Answer:
Hello. Thank you for writing to us

I have gone through your query and I have also reviewed the Image.

Your alopecia is quite progressed already i.e Norwood scale 4a. I don't think you can expect to regaine new hair just by medical treatment.
I would suggest a hair transplant for reconstruction of anterior hair line along with medical treatment which would include Topical Minoxidil and Oral Finasteride.

Regards

Can I go for a hair transplant at this age? As I heard that one can have a hair transplant only after attaining 25 years of age?

Brief Answer:
You can go for transplant at this age as well

Detailed Answer:
Hi.

Yes you can.
Though you are right in a way that generally patients are advised to go for hair transplant once there alopecia has evolved completely i.e is stable. The reason being that if one goes for hair transplant too early when alopecia is still very early Or progressing there may be new areas of alopecia/ thinning at places which had a good density earlier and therefore these patients may require another transplant at a later stage.
However, since your alopecia is already quite progressed on Norwood scale i.e 4a therefore you cannot expect any more progression thing to happen on your anterior and mid part of scalp which are almost bald. Therefore you can go for hair transplant at this age as well.
The only thing that can happen in your case is that you may continue to loose already exisiting hair from vertex part/ crown of scalp. Therefore, to preserve those hairs (vertex) you must continue with medical treatment i.e minoxidil and finasteride.
