Suggest Treatment For Severe Cough

Question: Hi
My son is 1.5 years old. He has been suffering from cough and cold for past 3 days. I consulted doc today and she advised asthalin syrup thrice a day with relent syrup twice a day. He has a bit of congestion too, and is coughing quite frequently since past 6 hours or so. He has got pretty quick compared to his highly active normal self.

By what you quote I feel what your kid could be having viral associated wheeze or multi triggered wheeze. I have a few questions for you -

Questions:
1. How many days per month does he cough or feel breathless?
2. How many nights per month does his sleep get disturbed due to above symptoms?
3. Does he feel breathless when he plays around?
4. Are the symptoms when there are seasonal changes?
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.?
6. Is the cough always associated with fever?
If your answer is yes for any of the above questions, your kid might be having viral associated wheezing or multi triggered wheezing and I suggest you meet a paediatric pulmonologist who is near your place.

Thanks for your response. Please find the answers as below for your queries.

1. How many days per month does he cough or feel breathless? - None. Its not something often. Now also he is not really feeling breathless, its more of a one off thing that he has cought cold.
2. How many nights per month does his sleep get disturbed due to above symptoms? - None
3. Does he feel breathless when he plays around? - No
4. Are the symptoms when there are seasonal changes? - We are based out of XXXXXXX so more or less the temperature does not have a huge alteration, so cannot commit on this. But yes generally when there is an extra dip in temperature he does catch cold. I would assume he cought cold a few nights back when temperature was pretty much on lower side at night time, and often he kicks his blanket off.
5. Is there any family history of asthma or any other sort of allergies like skin allergy etc.? - No asthma. I do have a few skin rash on my biceps.
6. Is the cough always associated with fever? - Generally yes there is a bit of fever associated to the cold.

He looks extra weak tonight after giving him Asthalin and Relent.

it looks like your son has got a viral associated wheeze. The prescription which you have mentioned seems to be fine. With this he should definitely get better in a day or two. But if i were your treating pediatrician I would consider adding montelukast as well. This newer drug addition I would consider giving for 7 days.
I suggest that you meet your pediatrician and ask her about montelukast and this is a prescription drug.

Tonight fever went pretty high. In fact highest in last 3 days. It reached about 102. Gave Crocin drops twice in an interval of about 5 hours. But why did fever go up suddenly. Point to note is there has been a definite improvement in his coughing. He has not coughed much tonight. Shall we take him to a hospital? On giving Crocin fever did reduce to about 99.

Viral illness take 3-5 days to settle

Detailed Answer:
Hi XXXXXXX .

1. It is quite common for fever in viral illness to shoot up suddenly like this.
2. No need to take him to hospital if he is active when the fever is down.

As I have mentioned that it could be viral associated wheeze, it means a viral illness is triggering a wheeze in your kid and this viral illness might last 3-5 days. I take this opportunity to tell you something about fever - when to worry and when not to worry -
1. Fever of few days without any localizing signs could as well a viral illness.
2. Usually rather than fever, what is more important is the activity of the child, in between 2 fever episodes on the same day.
3. If the kid is active and playing around when there is no fever, it is probably viral illness.
4. Once viral fever comes it will there for 3-5 days. So do not worry about duration if the kid is active (when there is no fever).
5. Paracetamol can be given in the dose of 15mg/kg/dose every 4-6th hourly that too only if fever is more than 100F. I suggest not using combination medicines for fever, especially with Paracetamol.

ANOTHER IMPORTANT FINDING IS THAT GOOD RESPONSE OF COUGH TO ASTHALIN IS AN INDERECT INDICATOR THAT IT IS WHEEZING.

Be rest assured if the kid is active when there is no fever and he will be alright in a day or two.
