Suggest Treatment For Severe And Persistent Headaches

Question: Hi i have had very bad head aches on the left side on my eyebrow, in the middle of my brow and the back of my head( left side) and then sometimes it switches to the right eyebrow and temple. It is very painful and i have had this for a week now. I am sensitive to light also so if i am in light for a long amount of time it worsens. Please help !

I read your question carefully and I understand your concern.

Judging from the location of the headache and its distribution the most likely cause does seem to be sinusitis involving the frontal sinuses. I assume that’s what you’ve already been told judging by your medication. Sinus infections are not always that easy to go away, may take 2-3 weeks to clear up. Panadol often is not enough to alleviate the pain, an over the counter anti-inflammatory drug like Ibuprofen is more appropriate and should be more effective. So you should take Ibuprofen instead of Panadol if you haven’t already tried it.

Another possibility would be that of tension type headache, the most common type of headache, often triggered/exacerbated by stress and anxiety. In alleviating the pain still Ibuprofen would be the best choice. Only when the pain becomes chronic may regular preventive medication with amitriptyline be considered.

Other more threatening causes like brain lesions are unlikely as they usually manifest other signs. However if the pain persists for over 2 weeks without improvement and you do not have a prior history of such headaches before then brain imaging should be scheduled to exclude brain issues. Such imaging includes the sinuses so should confirm the diagnosis of sinusitis as well.

Hi thank you !
I had a xray on my sinus but they said it wasnt infected as such but inflamed in my upper nose between my brows and was given a nostril spray whuch inhavent tried yet (first they gave me sudafed) - also the said i had a few pollops in mt sinus- would this cause this pain too ? my ears are even popping. And painful but when they look inside theyre not red nor inflamed, maybe pressure ? y

Thank you for the additional info. That reinforces the sinusitis hypothesis I would say. The polyps which you were told about do not cause pain in themselves, but they cause obstruction of the upper airways. As a results secretions accumulate which leads to inflammation and pain. May even be infected which is why antibiotics are used at times. It relates to the ear issue as well. The ear is connected to the upper airways by a canal called the Eustachian tube, when the airways are congested fluids from the middle ear accumulate and may cause distension and pain. Again may be infected the same way as the sinuses but probably not your case for the moment since you say there on exam color is normal.
In terms of management it consists in medication to relieve congestion and help the accumulated secretions to be eliminated. At times antibiotics are added when infection is suspected as in your case. As I said improvement may take 2-3 weeks. But it is not a threatening issue do not panic. Only in cases when the issues persists for long time or recurrs too often may surgery to remove the polyps be considered.

Thank you , i am using sensease just to see if it helps make my sinus feel better. I have tried si many things. So would iburofen take it away for good ? Or will i need to go on anti biotics.

Sensease is a good product, usually decongestants like pseudoephedrine, phenylephrine are advised as well. Lavage with saline solutions is also commonly used. As for antibiotics I thought that you were already taking one in amoxicillin...generally it is preferred to be waited if there is no indication of infection. If you develop fever, green nasal secretions or blood tests show raised white blood cell count, all these indications of infection, then antibiotics are indicated.

Regarding Ibuprofen I suggested it as an alternative to Panadol for the pain, it is not a solution, only to relieve the headache, hoping that in the meanwhile as sinuses are drained the pain will not return.

You mention to have tried so many things which makes me assume this issue has been present for more than the one week you initially mentioned. In that case if it is a recurring issue then you should consult an ENT (ear nose throat) specialist as perhaps removal of the polyps may be a solution.
