Suggest Treatment For Severe Acne

Question: Hai, I am 29 years old and has a history of severe acne eruption five months before. As part of treatment , I have applied sebaguard lotion along with some oral medicines whose names I dont remember now. The treatment was around for three weeks. Now the result is that my face, especially the cheeks are left with severe pigmentation marks. I have brought skinlite cream and clingard anti-acne gel. Pls help me with its usage since I am determined to get rid of this.Now its winter in the place I am in.

Hello,
Welcome to health care magic.
I understand that you have pigmented marks left behind by a severe episode of pimples. We call it post inflammatory hyperpigmentation.
Please throw away Skin lite cream as it will do more harm in the long termdand it is not indicated in your condition.
Wash your face twice a day with glycolic acid containing face wash like melawash or Ahaglo face wash. You may use clindamycin gel twice a day over pimples. Evening apply aziderm 10 percent gel over dark marks.
A gel form of sunscreen should be applied in the morning and noon over full face like photostable gel or Acne UV gel.
If you want faster improvement you may also consider doing a few sittings of chemical peel every 2 weeks after consulting a Dermatologist practicing cosmetic dermatology.

Hello Doctor,
First of all, good to know you are a Malayalee. Me too from the same place. Currently I am doing research in IIT XXXXXXX mentioning this because I will be in this place for quite sometime and the weather is harsh over here. This is really affecting my skin and frequency of hairfall. I have just done a surfing of the medicines you have suggested and my understanding is that they cause drying of skin. Its dry winter over here till Feb. And also I am not having acne presently, it erupted during extreme sultry summer in July, which I think has chances of recurrence. What I now have is only the dark spots. So I have my aspersions. Waiting for a further suggestion from you. Thank you.

If you are not having any pimples at present , you may avoid using clingard gel. Aha glo will be better than melawash for dry weather condition. As you rightly pointed out, there is always chance of acne recurrence and therefore the products that you use should be suitable for pimple prone skin. That is why I recommended Gel form of sunscreen as cream or lotion form of sunscreens have potential for acne formation. After washing face with face wash you may apply moisturizer, but again it should be suitable for pimple prone skin, sebaguard is fine, other options are Acnemoist cream or Papulex cream. Aziderm is a little drying, I agree but it is good for pigmentation caused by pimples. Another alternative which do not cause dryness is Kojic cream (Curatio) which you can use for pimple marks, even twice daily.

Hello Doctor,
Thank you for your suggestion. I have decided to pick on the kojic cream. Would you also suggest me the name of an acne UV gel, since there are a lot of them flooded in the markets, provided it is available easily, affordable and gets along with the kojic cream. I also would like to know if I can use this in the long run. I forgot to mention earlier that I have sensitive skin that is prone to rashes, irritation and itching when exposed to sun or dust. I would also like to know if consuming oily foods causes breakouts, since this is a fact that cant be helped in the winters.

Welcome back,
I had mentioned two gel based sunscreens earlier. Photostable is a good gel based sunscreen (Sun pharma). Other options are Acne UV gel (ipca pharma) or Sun mate gel cream with spf 30 ( Palson.'s pharma).They are suitable for pimple prone skin and can be used long term. They will go well with kojic cream and are suitable for sensitive skin.
You may use any powder on top of sunscreen, whatever you are using now.
More than oily food if you can avoid milk products like cheese, chocolates, pizza, ice cream .sugary foods like XXXXXXX Fanta, Pepsi, Tang etc it will be ideal.
