Suggest Treatment For Scaly Dandruff On The Scalp

Question: Dear doctor,



I have white scaly dandruff on my scalp (in clusters), some scaling behing my ears and on my forehead and 2-4 0.5mm diameter scales on my body. It seem liek I have psoriasis and my doctor has asked me to use a zinc pyridine shampoo and apply Tacrolimus 0.1%solution 0.1%. I am not sure if it is helping me, especially the patches on my face.



Is there anything else that I can do to help alleviate the white patches, especially on my face? Like home remedies, any specific soap, diet etc?
I am also on acne treatment currently- isotretinoin 10%, finast
eride, vovisun.

I am dr.kakkar(dermatologist). I have gone through your query and i have noted your concern.

The remedy you have been using is unlikely to provide any significant relief. Home remedies too are not advisable for your skin condition and may either prove inadequate to provide incomplete relief.

For scalp involvement you may use a coal tar and salicylic acid based shampoo e.g protar lotion Or Ionax T shampoo either daily or on every alternate days.

Use a steroid+salicylic acid based solution e.g topisal-3% solution or halovate-S solution on scalp for overnight application

Use eiher mometasone furoate cream e.g momate or elocon cream for forehead patches

What are the possible reasons for prescribing tacrolimus to me, then? I really dont think it's working.

Tacrolimus is less potent and therefore helpful in control of milder disease or for maintenance of improvement, once good control has been achieved with a topical steroid preparation.
