Suggest Treatment For Running Nose, Cough And Fever

default
Posted on Mon, 14 Nov 2016
Question: Hello Dr XXXXXXX
Hope you are doing well!

This query is for my mother 47yrs old. She had fever & feeling cold last week. In my village, doctor advised to take her some medicine for which she took 3-doses on last friday. I am sorry but she is aware of the name & doctor hasn't given any written prescription. That's the problem with the small town medical care.

Now there is no fever but there is running nose & cough. There is pain in the throat and while coughing & swalloing, it pains in the throat. Can you please advice some medication? Let me know, if you need more details to prescribe the medication.

Regards
XXXX

Detailed Answer:
Hello XXXXXXX
Good to hear from you again,
Yes I'm fine hope everything's well at your end too..

First of all, let me know specifically which medication she took..if she can remember the name and dosage, otherwise even the empty strip will be having it imprinted, just look at that or even ask the chemist of the medicine shop from where she bought it..

Yes, a few more details required i.e.

Is the cough productive, any phlegma, if yes what is the color of it ?

Is there any difficulty in talking / eating / swallowing ? any hoarseness of voice ?

Any difficulty breathing ?

What is the color of the secretions coming out of nose ?

Any headache / feeling heaviness over head / bodyache etc. ?

Does she have any significant past/present medical ailments (any particular medication is she taking regularly for any other health concern) ?

How is her general health overall ? When was the last time she got herself checked up and how were the results ?

Good to know there's no fever now, any other problem apart from the cough/cold/runny nose ?

Let me know all these details so that I can advice appropriately, this being online medical forum won't allow us to prescribe medications, so after I advice you can simply get them prescribed by any local physician ....

Awaiting..

Follow the Advises as below..

Detailed Answer:
Hello XXXXXXX Welcome back,
Thanks for following up with the details,

Now as per these descriptions, it seems like pharyngitis and/ tonsillitis (exact diagnosis is possible only after examining her clinically), however treatment modality remains almost same..

Fever is there due to infection, should be treated with appropriate antibiotic therapy..

I suggest my patients to be on Tab AZITHROMYCIN 500mg Once daily (with or after food ) for next 10 days (prescription drug, so please obtain it from the local physician)..

[it's a safe antibiotic, however, if at all any allergic reactions, palpitations, loose motions/diarrhoea (mild diarrhoea can be ignored though), excessive nausea/vomiting observed then please stop it and report it immediately to any nearby healthcare center]

Syp BROMHEXINE (numerous brands available, eg. BRO-ZEDEX, ASCORIL+EXPT etc.) for the productive cough 10ml three times daily for next 3 days then can be reduced to twice daily for next 5 days (depending on the intensity of the cough), make sure she doesn't eat or drink anything immediately after taking this..(preferable time should be after breakfast, lunch and dinner)..

Betadine gurgle three times daily for next 7 days.. also put a pinch of salt in XXXXXXX warm water and gurgle twice daily as an alternative fashion for next few days..

Tab PARCETAMOL 650mg two to three times daily after food for next 2-3 days for the fever and pain..

Cap PANTOPRAZOLE 40mg Once Daily in empty stomach (15-20 min before breakfast) for next 10 days to prevent any drug induced / underlying acidity..

Please get all these prescribed by any local doctor..

Steam inhalation three times daily (make sure to cover the head properly with a towel for 2-3 mins while doing this)..

Tell her to avoid cold drinks, dairy products, sour foods for the time being..

Make sure she remains well hydrated, drink plenty of water (~ 2.5 ltr per day) and XXXXXXX warm liquids (eg. herbal/green tea, soups, broths, porridge etc.).. avoid spicy foods and have simple home cooked meals for the time being..

There will be a lot of secretions involved after initiating the treatment, instruct her to buy extra pack of tissue papers and not to use handkerchief for the same..not to swallow the secretions, and continuously spit/throw them out..

With all these, she should be fine, however if at all observed any further worsening of the symptoms, recurrence of fever, then I'd strongly suggest to get her clinically evaluated by any Internal medicine specialist or to any nearby healthcare facility..

Do let me know how she stays in the due course..

Take Care
Kind Regards
