Suggest Treatment For Recurring Bronchitis With Acid Reflux And Obesity

Question: i have had chronic bronchitis for four decades.i am male and 70 years old.i am somewhat overweight.i have never smoked.i have not been exposed to industrial pollutants.i have good lung capacity and good peak flow.my resting pulse is stubbornly at 72.and regular.the exacerbations...one every four years or so,have been difficult.i have eliminated sinusitis as a factor,but only recently identified....empirically,gastric reflux as at least one other factor.the recent,more long lasting exacerbation i stopped within a fortnight using cimetidine night and morning.my gp has as a result prescribed a one dose equivalent.the cough is now absent,but some breathlessness on mild exertion remains.also,the exacerbation has left in its wake considerable weakness and lethargy.it was the most impactful ever.i have had a few episodes,say one night in seven, of waking due to shortage of breath.not, i think,a matter of p.n.d.,......there is no coughing,certainly no gasping or urgent need to sit up.certainly nothing that one would call a paroxism.also,uncharacteristically for p.n.d.,it has occurred late in the night.02 is at 99% and pulse at about 60 , of good volume,and regular during one of these episodes ,which last around an hour.breathing rate seems to be about 8/min.i do not understand why i can have the sensation of shortness of breath,when the 02 saturation is at 99%. perhaps you can help.i have none of the indicators of heart failure variously cited...the ppi killed the cough,so i know what caused that. my question is this....could this be an early sign of heart failure,or is it more likely to be part of the bronchitis presentation?

My opinion-
In the given situation-You seem to suffer from-GERD with acid reflux,with esophagitis, amongst many causes at this age. In view of the PND, gastritis with IBS needs to be ruled out. With submitted reports about cardio-respiratory vitals, heart failure does not seem to be cause of this bronchitis; but mostly is from acid reflux as it has responded to Cimetidine treatment.

Do's suggested-
Bland rice/potato liquid porridge
Sweet watery Butter Milk x 2-3 times a day.
Avoid spicy, nonveg,fried diet.Add more liquids to daily intake
Reduce in solid volumes and fats / nonveg -for 3-4 weeks time.
Yoghurt x 3-4 weeks.
Sleeping on left side with Head up position would reduce such attacks.
Reduce weight with low intake and adding 30 minutes walk / or jogging.
PPI- like Esomaprazole with domeperidone x 1 -2 times a day before meals x 2-4 weeks would be more resolving
Antibiotics /Anticholinergics if entero-colitis with IBS is confirmed from stool- ova/cyst, after Physicians and Surgeons check ups.
Concentrate on changing life and diet style as told above, to have long remissions from bronchitis / and bronchospasm resulting from acid reflux
No risk of fatalities if above care is taken.

EKG/Stress test done by a Cardiac Physicians to be considered if you worries about heart failure persists.

All the above medicines to be taken with script from attending family / Physician/ Surgeon doctor.

Further suggestions in this regard would need more information /investigations after getting more details from you.
