Suggest Treatment For Recurrent Canker Sores

Question: Hello Doctor. I had a thermal burn in my mouth last year due to excessive use of mouth wash and consumption of extremely hot food and beverages. With your suggestion last year it had healed by sucking on ice pops but it had taken a long time to go. Almost 4 months.

I had that problem back now. I don't drink hot water anymore but I accidentally burnt myself by hot tea and hot soup on the same day and the white scalded and raised gums were back. This was more than a month ago. I used the same recovery options as you had suggested. Dead skin starts peeling in 48 hours but the healing is extremely slow. I have been taking B12, vitamin c and folic acid supplements. Drinking cold raw milk, coconut water etc, but the only thing that has shown some promising progress is hexigel. I see the inflammation reduced visible when I apply it.

Everything was going at its place until 4 days ago, I ate some cold but crunchy and very sharp food and all the gums in mouth swelled and developed painful blisters overnight. And now I am not able to eat anything except for mashed banana or liquids as chewing anything as soft as boiled sago feels like chewing a bunch of iron nails.

I had consulted a very XXXXXXX dentist earlier this month and he had prescribed Phexin 500 mg and vizylac which hasn't helped at all.

My dentist has confirmed I don't have any plaque or gingivitis and it's a classic case of gum injury but no one has been of any help on how to heal it.

I would appreciate your help on this as eating has become a nightmare for me now. And I have to travel out of city for a conference in less than 10 days and there I won't have the option to eat mashed food. I'll have to eat whatever is provided.

As per your complain it seems to be a severe case of Apthous stomatitis or Canker sores...

The first thing that you should do is to continue to suck ice and do cool water rinses by holding cold water in your mouth for 15-20 seconds and then spit off..
Do atleast 4-5 times a day..

You should get steroid based mouthwash containing Dexamethasone and another preparation of numbing mouthwash containing Lignocaine...

You can gargle with numbing mouthwash containing Lignocaine before meals so that it will help in eating properly by numbing the mouth and reduce pain..
You can also apply Hydrocortisone containing oral gel over ulcers.
As of now you will have to take very soft and semisolid foods to prevent further damage..
Take anti-inflammatory painkillers like Ibuprofen...
Continue phexin and vizylac...

Also please indicate that what medication are you taking for low platelet and WBC count...

Hello Doctor, thank you for your response. I am not taking anything for low platelet and WBC count right now. Until last month I was on multi-vitamins for almost two months. Have tried supradyn and bevon.
Phexin and vizylac have not helped at all and the dentist already changes the medicines and has put me on oflomac dz and sumo from today.
I just want to know whether the gel and mouthwash you have prescribed is going to sting?
Also, I just wanted to clarify that these are not pus filled or fluid filled sores. They're rather when your gums are already sensitive from prior injury and you accidentally stab the soft tissue with tortilla chips and how it swells up and skin starts pulsating and becomes extremely tender to touch.
I have one more question, can I wrap an ice cube in a soft paper napkin and apply it on the traumatized area? Would it help?

As per your complain No, the mouthwash will not sting..

The condition you are describing will definitely improve with it..

No need to wrap ice cube, you can simply suck then or put cold water in mouth that will reduce inflammation and soreness...

Also once get the platelet and WBC level checked for knowing the exact levels and if there is any need for treatment..

Hello Doctor, I have found something called lidocam oral rinse that has benzoxonium chloride and lidocaine hydrochloride mouthwash written on the bottle. Is it the right product?
Also I am unable to find dexamethasone based mouth wash and hydrocortisone oral gel. Could you prescribe the exact product names?

You can look for Benzydamine mouthwash which can be available with names like Tantrum mouthwash, Difflam oral rinse etc..

Kenacort oral gel can be taken for application over ulcers..

The lidocam oral rinse is appropriate..

Hello Doctor, I have been trying things you suggested but I don't see any progress at all. Also, I wanted to say that these are more like gum lacerations than canker sores.
Are there any anti-biotics to take? How long do non-complicated lacerations take to heal?
Also, I have got rashes on my entire body including face, palms, hands, legs, neck everywhere when I woke up this morning. I did not take any medications yesterday except for applying kenacort. I don't have any known allergy to anything. Can kenacort be responsible for rashes?

As per your query rashes can be due to Kenacort but I have not commonly seen it to occur..

Secondly improvement will take time and does not occur so soon..

- Antibiotics are not needed until infection is ruled out..

Also I would like to know a few details like:

-Did the rashes occurred immediately after applying Kenacort?
- Is there itching also on the rashes?
-Are you gargling with Lidocam oral rinse?
