Suggest Treatment For Recurrent UTI And Enlarged Prostate

Question: Hi I am 56 years male and I some four years back developing shooting pain in my left flank with chills and fever . I was advised to go for USG which showed multiple stones of 3-4 mm in my both kidneys and prostate enlargement of 32 gms. I was advised to take Levofloxicin 500 mg for 2 weeks with plenty of water daily . Everything was ok but after that again in 2012 same problem occurs with severe pain during urination , was advised for culture sensitivity and theee was E. coli presence with sensitivity for nitrofuratoin so given Neftran for 2 weeks and again everything was normal but after doing another USG my prostate was 36 gms was advised to take silodos-D8 which I am taking now for last six months. However I never experienced urgency for urine during night hours but my urologist said continue silodol d8 it will help you in long run. But in March this year I again developed UTI with severe pain while urinating but while urinating I could see a little clot coming out and few drops of blood as if something came out through my urithra it was hell of pain that time and after that frequent urgency for urination for about 8 hours went to urologist again advised for urine test and culture sensitivity. And USG again showing few stones 3-4 mm in rt kidney and left free of stones.
E.coli again with sensitive to penems and nitofurntoin and amikacin was again advised to take faropenem 300 mg twice a day for two weeks and after that nitrofurantoin 100 mg daily for 1 month. I was absolutely ok with urine force showing significant improvements as silodos d8 continued. Yesterday I was absolutely normal without any urinary symptoms but during night time I suddenly awoke for urgency for urination. Again there was normal flow of urine but could feel something causing discomfort in urination I pushed a bit hard to expel few drops left behind but to my XXXXXXX again a little clot came out with shooting pain and 2 drops of blood which caused a little of comfort but again after few minutes urge for urinatin subsided but could find in morning no blood drop but clear whiter drop by pushing my urithra which continued for whole day before urinating. Collected sample for examination again got report 40-60 puss cells 0-1 RBC rest was ok now waiting for culture sensitivity report. Rest there is no urgency for urinating but fever is 101 degree . I called my urologist he has again advised to nitrofurantoin 100 bd and silodol d8 continue. He said it can be chronic bacterial prostitis. He said better to go for TURP . But I am worried. What should I do please advice

Thanks for your query,based on the facts that you have posted it appears that you are getting repeated episodes of UTI since last 4 years mostly due to bladder outlet obstruction due to Enlarged Prostate .

Assuming that you have undergone all relevant tests under guidance of your Urologist and confirmed diagnosis of Enlarged Prostate in view of repeated episodes of UTI he has rightly advised you to undergo endoscopic surgery for resection (removal ) of prostate (TURP)

This is a gold standard and simple surgery practiced all over the world for resection of enlarged prostate and gives very good results and helps to improve quality of life and prevents getting repeated episodes of UTI.

Do not get scared about the surgery .Go ahead and get it done .

Tell me the side effects which can occur after TURP? and also let me know how long it takes to perform the surgery and stay at hospital with recovery time after surgery? Also suggest best doctors/ hospital in north XXXXXXX preferably XXXXXXX where I can undergo TURP.

Retrograde ejaculation is the known side eefct of of TURP.

Thanks for follow up

It takes 1 hour to for surgery and one has to be hospitalized for 3 days .

As the bladder neck is resected along with prostate it causes side effect of Retrograde Ejaculation .However it does not affect sexual function or Erectile function and getting climax (Orgasm ) of sexual satisfaction .
