Suggest Treatment For Rapid Heart Beat And Tingling Sensation In Left Arm

Question: I have this problem of rapid heart beat and tingling in left arm while I sleep for last 2-3 days. it gets me wake up every time I experience Rapid heart beat And Sometimes feel chest heaviness in day time. Previously I have a history of Allergic rhinitis (my IGE level comes 637) I'm using Montek LC for that. Do I have any heart problem ? I have had EKG and 2D Echo thrice and one very recently and they came normal and Doctors strike out any abnormality in heart though AMVL prolapse Level 2 was in report and I also have D-Dimer level of 534 but Doctors have rejected any possibility of blood clot as there was no evidence of a clot in 2D Echo.
My question is this a sign of Heart attack ?

Hi, I am Dr. Mody and I would be addressing your concern.
Now of all the history you have provided I think the culprit here is Mitral valve prolapse. Before I start I would like to tell you that you don't have a heart problem in terms of blockages and stuff, it's just a little loose valve which is causing all the trouble.
Now in mitral valve prolapse, ectopic beats and some transient arrhythmia are very common . These ectopic beats would make you feel the sensation of palpitations , hereby a small dose of a drug should be good enough to give your relief from the symptoms as well as it will help in preventing the grade 2 mvp to progress any further. However before we start you on any medication, we need you to document the same.
So you need to get an EKG during the symptoms, which is nvr possible in most cases, so the best test here is to do a 24 holter test, and during those 24 hours you need to do everything that you usually do, like walking, going to gym, parting whatever you do, so if your symptoms occur everyday this would be the best test to document things.
As such your rhinitis and even montek lc wouldn't cause any racing of heart. However a drug like deriphylline, asthalin which are very commonly in rhinitis may surely increase would heart rate.
D dimer value too wouldn't suspect anything significant in your case
Went through your echo, both report s show mvp, the previous one had that you had septal hypertrophy which is a completely different entity, however the measurements mentioned doesn't fit into the same, so maybe it's a misdiagnosis and we may just avoid it. However I would like you to rpt a 2d echo after 1 year and than every 3 years

Thank you for the detailed reply.The two things I would like to hear from you.

1.Thank you for reminding of Mild Septal Hypertrophy, As you pointed it out, it was mentioned in my First echo report. So I went to a different hospital in XXXXXXX for my echo again very recently. The Doctor there dismissed any possibility of Septal Hypertrophy and HCM. As you already stated that it may be a case of misdiagnosis,I'm sending you the 2nd page of recently conducted Echo (where all the measurements are mentioned ) So that you can let me know what exactly it is.(File name saved as Recent Echo 2nd Page )

2. Secondly these heart Palpitations are so bothersome sometimes that I find it difficult to concentrate and even sleep sometimes.How can they be treated and cured ? What medications you would suggest so that it can ease my rapid heart beat ?

i went through the echo report , all measurements are normal , don't worry about the same
> for the mvp and palpitation in you , my first choice would be tab metoprolol 12.5 to 25 mg , however the dose would be decided on the symptom relief and your vitals , importantly the blood pressure. Do discuss the same with your doctor
> now 24 hotler is the ecg recording for 24 hours , which would record while you perform all the daily activities and we may corelate it with the symptoms by time .
now based on that we may start you the above drug and titrate the dose .
sometimes it happens that during the symptoms the holter recording was normal , it means the symptoms are subjective and no treatment needed for the same , as rarely MVP is know to cause panic attack like situation.
> if any problem i will arrange the holter test for you in XXXXXXX , its not really costly , and will help us objectify our treatment
lastly MVP is very common , like 2 out of 100 ppl , so just don't worry about the same as it does not increase any future risk of heart attack or any heart related significant problem
