Suggest Treatment For Presence Of Epithelial Cell In Urine

Question: My daughter is having nephrotic syndrom because of protein going with urine and her recent urine test reports have shown presence of granular casts (+) and epithelial cells (+) in urine. what does it indicate? what should I do now? please help asap. I am very much worried.

Nephrotic syndrome is very common in this age group, I see them day in and day out. Most of them do very well so please do not worry about it. It is treated with steroids. However I need to know a few more details

1. Was there any fever, sore throat or rash prior to the syndrome ?
2. Is there any family history of nephrotic syndrome ?
3. is she on any medications or has any medications been started ?

1. yes, she had a fever followed by swelling on eyes and sore throat.
2. there is no family history of such illness.
3. 2 years ago she took wysolone tablets and after some days problem disappeared but now it has relapsed. She is now taking omnacrotil 40 and having some side effects. recent urine reports have show presence of granular casts (+) and epithelial cells (+) in urine.
what should i do now sir.

Hello

You seem very tensed. Please don't worry, most nephrotic syndromes disappear by the age of 12 to 16 years. Here is how you should go about it

1. She has not had nephrotic syndrome again for a long time and this implies good prognosis
2. According to guidelines wysolone has to be given in case of relapse for 2 weeks, at the end of two weeks of the urine shows nil albumin then wysolone is tapered and stopped
3. Try and avoid high protein diet, typically found in red meat
4. low salt diet will help reduce the swelling
5. watch out for throat and skin infections in the future
6. Do not worry about epithelial cells and casts, they are part of any urine sample with proteinuria
