Suggest Treatment For Piles And Anal Fissure

Question: Q.I've been having this problem for a month now. When I first got it, it just happened straight away. Everytime I go to the toilet it burns severely like I had been drinking acid. On my 3rd day of having it, I had blood dripping out every time a small amount of stool came out. I went to one doctor where she inspected the outside and inside of my anus and told me I had a Hemorrhoid cluster inside, and one of them had bled during a bowel movement. We sent off a stool sample to get tested to see if I had a "bug" in my bowel that was causing this burning sensation. But it came back negative and the Doctor pretty much told me there was nothing else we could do. I didn't believe this, so I went and saw another Doctor for a second opinion. He too inspected the outside and inside of my anus. He mentioned that I had a couple of Anal fissures and that was what was causing the burning. Is it possible to get anal fissures and not feel it? At the beginning, 3 weeks ago, my stool was soft, I had no trouble pushing it out and I didn't feel any part of my anus tear. But the past few days my stool has become hard at the beginning and then turning very soft at the end. During these bowel movements I have felt the tears as I've tried to push out the hard stool. Then when the soft stool passes through, it burns like crazy expecially where I've torn . Do anal fissures cause buring around the WHOLE anal area and even about 10cm inside the anus? Or does it burn just in the area where you have torn?

Welcome to health care magic,

First, if you were having slight blood in your stool during bowel evacuation and if one doctor has examined per anum then yes it might be because of internal or external haemorrhoids.It needs to be rechecked by proctoscopy test by a general surgeon if you have still any doubt.

Anal fissures are nothing but the ulcers as a result a tearing of tissue around anus which causes burning sensation during and after passing of stool.

The reason could be your hard stools passing for the past few days which actually causes excess straining during defecation which in turn lead to tearing of the anal skin or mucosa.

These fissures or ulcers causes burning sensations around the anus and anal canal and haemorrhoids might cause the blood during stool passing.

You should go for routine blood with electrolytes, stool examination (you have already done before) with proctosopy by an experienced surgeon to have a correct diagnosis and the positions of piles.

You may require,

1) Antibiotics (quinolones like ofloxacin or norfloxacin with antiprotozoals like metronidazole or ornidazole) with
2) Topical application of steroids with lignociane creams or ointments to reduce pain and inflammation with
3) Hot seitz bath,
4) Laxatives or purgatives to have a soft and effortless stool evacuation.
5) Vitamin b complex with folic acid to have a rapid healing of ulcer.

You must consult general surgeon for complete history, thoroughly examination and treatment for suspected piles and anal fissure.
