Brief Answer:
Considerations

Detailed Answer:
hello and thank you for submitting your question.

I am sorry to hear that you have not had any relief of your symptoms despite extensive office visits. You mentioned that you have seen a specialist, which hopefully includes an ENT (otolaryngologist) Doctor. If not, this is a physician who specializes in this upper head & nexck anatomic area. Also, you may need imaging studies and or direct visualization of the sinuses with a scope. Unfortunately, it is impossible to tell what in particular the next step is without a physical examination. However, if you have not yet had any of these other procedures, it may be worthwhile to ask about their utility in diagnosing & managing your condition. Since you seem to not respond to nasal sprays, which may have contained a steroid, it is quite possible that you may have some sort of anatomic obstruction caused by polyps or hypertophy (overgrowth) of the tissue in your sinuses. Again, this requires a physical exam & possible adjunct studies for a definitive diagnosis. I can understand your frustration with the persistence of these symptoms, however it would be reasonable to return to a specialist (or seek a new one) & clearly tell them that you have not responded (=have been refractory to) to the previous nasal sprays & would
like further work up of this including imaging studies if necessary.

Sincerely,
Dr Galamaga
