Brief Answer:
it can be cervical spondylitis

Detailed Answer:
Hello,
I have studied your case. Shoulder problems usually not causes pain in whole hand. Rather pain is limited to elbow. I think your pain is due to cervical spondylitis. In this condition there is nerve compression leading to pain from neck to whole shoulder ip to fingers associated with tingling and numbness.
It can be confirmed by x ray of the spine. I have following suggestions for you.
1. Avoid using very high pillow.
2. Tablet Pregabalin and methylcobal daily.
3. Physiotherapy
I hope these steps will help you. Let me know if there is any other followup questions.
Thanks
