Brief Answer:
2 things: 1. consider surgery to treat current conditions, 2. consider IBS

Detailed Answer:
Hello sir and welcome.

Thank you for writing to us.

I have gone through your query with diligence and would like you to know that I am here to help. I understand how you must be quite frustrated with your symptoms, but there are a few things I need you to know:

+Hiatal hernia: As oral medication does not seem to be helping you much, I would recommend you discuss considering surgery with your doctor as it can really help

+Bleeding ulcer is a cause for concern, and should be dealt with using surgery at the earliest

+But I would also like to point out that almost all your symptoms point towards another condition which I am guessing, namely irritable bowel syndrome (IBS)

Most of your treatment so far has not proven to be helpful, which could either be because:

+The treatment approach is not correct, do consider surgery to help resolve your current diagnosis
+The diagnosis is not correct; please consider discussing irritable bowel syndrome with your doctor or write back to me if you wish for more information

I look forward to hearing from you in order to help you further. Please do not hesitate to write back to me for any further clarifications, I am always here to help.

Best wishes.
