Brief Answer:
You need immediate medical attention;should go to ER if not your primary

Detailed Answer:
Hi my dear,

I am sorry for the health situation you are going through.

Having a productive cough for 26 weeks associated with dull ache behind your right shoulder blade are indicative for pneumonia (respiratory infection).

You are reporting to have run xray, can you please send the report in so I can analyze the findings myself?

Smoking and drinking alcohol are aggravating factors for your actual respiratory tract problems and will delay the healing process.

An immediate consultation with your primary physician or pulmonologist is recommended as you might need to take prescription antibiotics or might need hospitalization. If you can't get an immediate appointment should go to ER.

For the moment, I advise to:
- increase fluids intake (water, tea, juice)
- take OTC cough medications
- antihistamins can also help
- apply evaporating balm on your chest and back
- take anti-inflammatory meds

Wish fast recovery!
