Question: Is it odd to have 9 perineural cysts? I have 7 on my cervical spine and 2 on my sacrum

Perineural cysts in the spine are not worrisome. These are fluid-filled sacs that form on nerves at the base of the spine. A person with such cysts would likely never know it. They almost never cause symptoms.
The pain that you are having is likely to be due to nerve root compression at the level of sacrum or lower spine. This can be due to a mild inflammation around the nerve roots. Such neural pains can be relieved with methylcobalamin supplements and medicines like Pregabalin. Consulting a neurophysician will help.

What is your opinion of this mri? Considering c4-c6 fusion but very hesitant after infection following previous surgery. Have severe headaches. Getting botox injections but not really helping. Severe neck and shoulder pain .

I have seen the attached MRI scan report. The degenerative changes in the cervical spine are mild one and can be managed without a surgery.
Initial Treatment options include Pain Killers and Physiotherapy.

If this is not successful, then, surgical decompression of the disc will be required.
There are various treatment options like
1.) Wearing a neck brace
2.) Physical Therapy
3.) Traction
4.) Steroid Injections
5.) Surgical Correction.

Follow these lifestyle modifications,

1.) Apply heat or cold packs regularly.
2.) Take a hot shower everyday.
3.) Do regular physical therapy.
4.) Stick to your treatment plan and go for regular follow up.
5.) Avoid high calorie foods and eat healthy nutritious balanced diet which should include fresh fruits and Green leafy vegetables.
6.) Exercise regularly and maintain a healthy weight.
