Brief Answer:
Local gels, medications, direct examination, biopsy.

Detailed Answer:
Hi,

Thank you for your query.

1. You can apply medical grade liquid glycerine locally. OTC preparations such as Zytee, Orasep, Smyle contain an anesthetic agent which lessens the pain.

2. It is better to get the ulcers checked as they are growing in size and since he has a history of using Paan XXXXXXX for a long time. You can also upload a close up picture here. A dental check-up and a biopsy from the edge of one of the ulcers can be done.

3.An anti-inflammatory such as Disperzyme will help. Muciane Gel and anri-reflux medication should be given. A multi-vitamin with zinc supplemet should be started.

4. As the sores are chronic and non-healing, a direct examination is recommended.

If you have any more questions I will be available to answer them.

Regards.

Brief Answer:
Get a biopsy done as soon as possible.

Detailed Answer:
Hi,

Thank you for writing back.

1. From the images, he definitely needs a biopsy (or preferably multiple biopsies) from the edges of the ulceration on the left half of the tongue.

2. The lesion is fairly large. Multiple areas of leukoplakia (white areas) are seen. Dental staining is seen and oral hygiene is poor). This definitely looks precancerous or cancerous. Only medication will not help, except to reduce the pain.

3. Kindly upload a biopsy report.

If you have any more questions I will be available to answer them.

Regards.
