Question: I fell a little over a month ago. I lost my balance and fell all of my weight... Approximately 230 lbs , landed on my left buttox. I was sore for a few days but ever since then it hasn't gotten better, only worse. I cannot sit or lie on that side for very long or the pain is excruciating. Walking for long periods or going up stairs is torture. I tried stretching a bit tonight and it felt like knives in my hip. What's wrong?

Brief Answer: Needs management Detailed Answer: Thank you for asking! History of fall with resulting pain not relieving is a very common complaint in such presentations. Such traumatic falls need to be addressed ad evaluated for any slipped disc, compressive neuropathies, hip bone fractures, or just muscle spasms secondary to trauma and inflammation resulting in soreness. I suggest you should get yourself evaluated thoroughly by an orthopedic specialists by detailed radiological assessment and neurological assessment of compressive neuropathy. Once any bone fracture or compressive neuropathy or any slipped disc is ruled out the next step will be by a rheumatologist for symptomatic treatment with some strong pain killers and skeletal muscle relaxants like tizanidine, baclofen and orphenadrine etc. Mean while massages, posture modifications and continuous physiotherapy by trained therapists will help relieve the pain early.Stay mobilized and do as directed by your doctors. You need rest, your muscles need rest. It will resolve gradually and not in one day by continuous compliance and therapy exercises. Discuss different options of pain management with your rheumatologist. Now last but not the least issue of balance loss needs to be addressed. To sort out whether it was syncope or some vestibular dysfunction, you need to get your cardiac profile (EKG, perfusion etc) and vestibular function profile assessed as the cause was this. Also losing weight is what you need the most. Over burdened buttocks with weight lifting of the heavy body itself is a soreness to the buttock muscles. Get lighter by regular exercises, FDA programs for weight loss and good balanced diets. In nut shell, rest and triage of orthopedist, rheumatologist and physiotherapist with some detailed work up for any anatomical derangement and work up for delayed recovery is advised. I hope it helps. Don't forget to close the discussion please. Regards
