Looks to be deep decay involving nerve.

Hi.
Thanks for the query..

It looks like due to decay progressing since long it has reached the nerves present deep in the tooth causing nerve irritation and pain.

You should get an x ray of the tooth done.

If tooth is in a position that it can be saved then Root Canal Treatment can be done followed by a course of antibiotics and painkillers to control infection and pain..

But if the infection and decay has reached deep to the roots then the tooth has to be removed..

Please upload your x ray so that I can guide you with the most appropriate treatment..

For relieving pain as of now you can take painkillers like Paracetamol, Ibuprofen, diclofenac etc..
