Question: When I go to toilet, I see blood also falling and it is very painful in the anus. I also feel some sort of skin hanging out which is painful when touched.

I have read through your query and your symptoms seems to be consistent with anal fissure.
Anal fissure occurs when minor trauma occurs over the anal passage while passing hard stools, recent bouts of diarrhea or could be an existing tear following child birth.
Minor anal fissures would heal in some given time, whereas, major ones may require a surgical intervention. I would suggest you to consult with a doctor for a proper digital rectal examination so that the exact location and the extend of the tear could be found out and managed accordingly.
You are required to do the following meanwhile,
- take stool softeners for a few days, so as to avoid further damage by passing hard stools
- consume high fiber foods like green leafy vegetables, oranges, strawberries, whole grain cereals, yogurt
- apply Anovate cream or Anusol- HC, which contains an analgesic and a local steroid to help heal the tear faster. But I would suggest applying this after examination by a doctor so that you may know the exact location of the tear
- a sitz bath- sitting in a basin of warm saline water twice daily is sure to give you enough relief
