Question: Hello. I asked this question here and someone replied with Proctitis. I was doing some heavy lifting by putting together a squat rack on my own. The next day i started feeling some pain on the anus area. Peeing or going number two does not hurt. But pushing out the number 2 hurts. Stopping the urine stream hurts. Coughing or laughing hurts the anus. Basically any clenching hurts the anus. When i feel and press, the pain is felt right above the anus but not on the anus. The pain deacribed earlier feels like its coming from the anus though. Walking in smal steps doesnt hurt but any bug movements or steps hurts on the anus. I had an xray and was tols my coccyx bone is fine.

Brief Answer:
Sphincter muscles strain or edema or hematoma.

Detailed Answer:
Hi.
Thanks for your query.
Noted the history and understood your concerns well.

Please give some additional information:
When did you do the heavy lifting?
Any medications tried so far?

As per the detailed history, you have provided, the cause seems to be sprain of the anal sphincter muscles as the sphincter muscles are very tightly pulled together during any activity when there is raised intra-abdominal pressure. This is an involuntary act that occurs every time one strains, lifting the heavy weight increases the tightness. There may be an associated hematoma.

Hence I would advise you the following:

First of all, stop any activity that increases the intra-abdominal pressure, preferably take bed rest for the time being.

Sitz bath in a tub with warm water.

Take an anti-inflammatory medication with analgesic properties like Ibuprofen if it suits you.
Take a mild laxative to have soft stool.

Afterward, do consult a General surgeon or a Proctologist for clinical evaluation, per-rectal digital examination, and request for further investigations like high resolution Ultrasonography, preferably an MRI as it will show the exact cause, edema and/or a clot of the affected muscles or so and helps in planning further management as needed.

I hope this answer helps you, please feel free to ask for further queries if you feel that there is a gap of communication.

Regards

It was Tuesday of last week May 2nd. I was putting together heavy equipment and moving it around by myself. It got worse by Saturday probably due to me still doing cardio/ab exercise. I thought i might have slightly pulled something but by Saturday until today the pain has been pretty bad when i clench.

No medication so far. Just soaking in Epsom Salt and heat pads but it doesnt seem to do much when I clench/flex to stop urine stream.

The heat pads give a little bit of limited relief when walking. But any sudden movement, stopping urine stream, pushing #2 out still hurts. Its like a quick sharp pain in what feels to be at the anus.the pain usually goes away soon after or sometimes 1-2mins after.Any recommendations on what can help with that pain? Or is that something that i would just have to wait out?

Brief Answer:
As detailed below.

Detailed Answer:
Thanks for your feedback.

As expected the problem is there for the last almost 10 days and has lingered on indicating the need to follow what has already been mentioned above.

This will help you to get a relief as well as get the most proper diagnosis to get any additional management as found on clinical evaluation and investigations as mentioned above.

Please take the proper management and let me know how you stay afterward.
Take care.

Hello. Just writing this now to get another opinion. On Friday i went to a doctor and he said that i might have an internal abscess. He was going to give me an antibiotic shot but when i mentioned i had a slight fever the night before he refused to give me a shot and asked that i go to ER. I went to another doctor instead and he gave me aniti inflamatory and antibiotic shot. Within 40mins to an hour i was feeling much better. I could actually walk and sit. When i flex to stop going #2 or when i flext to stop urine stream it still bothers a bit but now it is a tollerable sore feel vs before where it felt like a strong shooting pain. Im just curious if this sounds like it could be an abscess? On friday it was difficult for the doctor to feel around since it was extremely tender. Today i did ome feeling myself. The anus does not hurt but rather the sore feeling is felt above the anus and i can feel a slight bump. XXXX

Brief Answer:
As detailed.

Detailed Answer:
With the description you have provided, this looks to be an inflammatory process, and most probably an abscess as there is fever too.
Continue the basic treatment as I have mentioned and the additional medicines as prescribed by your Doctor.
Also consult a General Surgeon ti get the most proper evaluation and management.
