Brief Answer:
It can be a soft tissue injury

Detailed Answer:
Hello,
I have studied your case and I think that you are having shoulder rotator cuff tear. It can be due to injury or degenerative change. But before I confirm this diagnosis I want to know little detail about your illness.
1. Since when you are having this pain?
2. How did this pain started?
3. Is this pain is also associated with shoulder stiffness?
4. Did you got MRI of the shoulder?
Please send me answer of these questions. I will try my best to help you.
thanks

Brief Answer:
You need to get MRI

Detailed Answer:
Hello,
Shoulder is a complex joint and it is must for us to have a MRI to see what is happening inside the shoulder. It can be a labral tear or rotator cuff tear. Just physiotherapy is not going to help you. So my advise to you is that you get a MRI as early as possible and then send the report to me.
Thanks.
