Suggest Treatment For Pain In Roof Of Mouth

Question: A few days ago the roof of my mouth began hurting. I have that whole torus palatinus thing, but I noticed that on the right side there was what looked like a canker sore forming. It is getting worse and it hurts! Now it looks like a yellowish oval. It hurts to eat or drink, and now the right side of my teeth are hurting. What does this sound like, and how should I treat it?

Hi. Thank you for writing to HCM.

Torus palatinus is a very common precipitating factor for developing an ulcer or a canker sore in that region. Due to the uneven surface of the palate there is an increased chance for injury or for repeated ulcerations and also a longer course for healing. Based on your description it sounds like the ulcer may be either non healing with bony involvement or is infected with a bacterial pathogen. This infection may have spread to the teeth on the right or if there is bony involvement the entire right sided dentition may be painful.

In my opinion it is recommended that you see an ENT specialist or an oral pathologist/dentist to have this evaluated. It is important to gauge the depth of the ulcer.

If it is superficial, it may recover easily with oral antiseptic rinses, topical antibacterial gels/ointments, oral antibiotics and anti-inflammatory medications. Multivitamin supplements ( Vitamin B complex and C) may help in early recovery.

If it is deep with bony (palatal) involvement then it may need debridement and histopathological analysis with a punch biopsy to rule out any midline granulomatous disease.

May be for now, till you see a doctor, you can start with chlorhexidine or dilute betadine oral rinses three times a day after meals and start your self on multivitamin supplements once daily. Avoid spicy diet, smoking or alcohol and drink up to 2 litres of fluids every day. See a doctor as advised at the earliest.
