Suggest Treatment For Pain In Back And Waist

Question: Hello doctor,

I'm experiencing some strange pain on my back, waist and abdominal region for a while now. The symptom is on and off, so sometimes I have a good day, sometimes I have a bad day. Yesterday was a typical day that I wasn't feeling well.

In the morning usually everything was fine, however starting at noon, when I was driving I felt that my back and waist, whichever body part was touching the car seat is feeling a bit hot and cramping/gnawing pain, however the feeling was a bit vague, I would rate as 2/10 on a scale. In the afternoon I went to a movie theater, when I was sitting in the very soft sofa seat, I felt quite stronger pain (about 5/10) on my waist touching the back support. I had to change my posture every 10 minutes to make myself feeling better. However, since I couldn't stand up for like 2 hours, I felt pain on my waist (left and right), middle back and under shoulder blade. I also felt pain on my abdominal region (especially both side under/between the ribs). After the movie is over, when I stood up I felt much better.

So these days it seems like when I sitting on a soft seat / sofa, I feel obvious pain, however when I sit straight, or stand up, I feel better (I can still feel vague pain sometimes however). Lying down or during sleeping usually won't cause any trouble.

Some of my questions... :

1. What caused my pain?

2. Why did the pain also go to the abdominal region?

3. Can I still go to the gym?

Hi, I had gone through your question and understand your concerns. The pain you felt, inconstant in nature, changing with body position, coming and going, and its localization leads mainly to pain caused by abdominal organs ( most likely from gallbladder, pancreas). However it is not fully excluded that pain may be caused from a thoracic vertebral column. Pain goes to abdominal area because nerve distribution. About going to the gym it is necessary to wait identifying exact cause of your symptoms. So I suggest you to consult your primary care Doctor and discuss above mentioned possibilities and further diagnostic tests needed. Hope this answers your question. I had several blood work and ultrasound this year for chronic upper right abdominal pain which comes on and off. And most resulted showed normal so the gastroenterologist considered to be functional pain. (Pain on the back is something new these days). I've attached my lab report. I had chest spine x-ray and it showed normal.

Also from the ultrasound on this April:

“The liver echogenicity is felt to be within normal limits. No focal hepatic masses or intrahepatic biliary dilation. Multiple gallbladder polyps are again demonstrated, largest 6mm, previously 5mm. No gallstones, gallbladder wall thickening, nor pericholecysic fluid. The common bile duct, spleen, kidneys and visualized pancreas, aorta, and IVC are unremarkable. There is a stable accessory splenule.”

Hi again XXXX. I read carefully all your data regarded to previous medical history. Probably your symptoms are caused by polips, but this can happen if they are located near secreting duct of gallbladder causing an on/off effect on it. Another cause to your symptoms may be a problem within your spinal cord at thoracic level and that needs in my opinion further investigation with a thoracic column MRI. So I suggest you to discuss this with your treating Doctor. Hope this helps. Wishing you good health.
