Suggest Treatment For Pain Due To Hairline Fracture On Foot

Question: I twisted my foot while walking on a slope area on 11th XXXXXXX This resulted in a hairline fracture in the base of the 5th metatarsal in the right foot. This was diagnosed post an x-ray on 12th XXXXXXX and a boot cast was fitted by the orthopaedic surgeon the same day. He advised immobilisation only for 4days though. The pain was intense when I joined work after 5 days so I kept away from for almost 13 days though. I must mention that post joining I had to climb 2 flights of stairs twice daily and walked around 150 - 200 mtrs total everyday. My orthopaedic was aware of this but he advised me to go ahead. The cast was removed yesterday after 30 days and a crepe bandage applied. Post that the spot of the fracture is swollen and there are continuous spasms of pain. I've been advised to walk normally without crutches but the pain intensifies on doing so. No physiotherapy has been advised. I've been prescribed calcium (triozac) and vitamin D supplements. For pain killers I have been advised enzoflam and dynapar qps spray. How should I proceed?

There is possibility of delayed union of fracture.
I will advise to continue crepe bandage and warm water fomentation to reduce swelling.
Continue your medication.
You can add tab union or boneheal daily twice.
Take rest and limb elevation for few days.

I have uploaded the first day x-ray photos and the ones taken just before the cast was removed (those are the ones where the fibreglass plaster is visible).

Some other questions I have are:
1. What is fomentation? I have swelling in the spot of the fracture so, is that advisable in this case?
2. Please advise after seeing the images if additional medication (boneheal/union) that you advised earlier is required.
3. For how many days would you advise rest and elevation?
4. Once I do start walking how much pressure on the foot and how much distance is advisable?
5. Is any physiotheraphy required before moving to complete movement?

I have seen your x ray.

Your post cast x ray does not show much fracture union so you should continue tab union/bonheal.
As you have swelling leuk warm water fomentation [warm therapy] may help.

Rest and elevation can be given for 4-5 days according to symptoms.

If process of union is there then you can keep full weight ,distance you can increase gradually.
Only ankle and foot exercises are sufficient ,no need of additional physiotherapy.

How is swelling now? I will advise one repeat x ray after 3 weeks of cast.

Hello Dr. XXXXXXX

The swelling and pain is a little better once I decreased the pressure on the foot. I have started using the crutches again and that helped, I believe, since the full body weight is not on the foot.

The orthopaedic surgeon I earlier consulted has removed the cast. Should it be re-applied?

Also, by when would you advise I start the ankle and foot exercises?

As per your x ray fracture pattern there may not be need to re apply cast again.
Use of crepe bandage may be sufficient.

If you can send clinical photo of both your feet then I may able to comment on swelling and inflammation.

If pain has reduced then you can start ankle and foot exercises with in 7 days.

I have uploaded photos of straight and oblique views of both feet.

For how many days should I take Boneheal/Union and should it be before meals or after? Please send me the complete names along with manufacturer so that I can get it from the pharmacy.

Are the above tablets also calcium supplement? I am as of now already taking triozac once a day.

One final question. Would you suggest I can go to work if the pain and swelling subsides in a day or two? Like I mentioned earlier, going to work will involve a minimal movement of climbing 2 flights of stairs twice daily and walking around 150 - 200 mtrs total everyday.

There is swelling over foot and toes,you will need rest for early recovery.

You may need to take union[Aeran pharma]or bonheal[akumentis] after meals for around 1 month.
These medicine contains extracts for early bone healing,you can continue your calcium along with it.

Yes you can go if pain and swelling subsides.
