Brief Answer:
You need a broad spectrum Antibiotic

Detailed Answer:
Hi
I saw the picture.

Redness is due to infection and inflammation of the area.

The pic doesn't typically fit with the diagnosis of Pilonidal cyst. Perianal abscess with cellulitis has to be excluded. If perianal abscess is diagnosed by a Surgeon,it should be drained.
I prefer to admit such people in hospital or at least daily nurse administration of Intravenous Broad spectrum Antibiotic along with Anti inflammatory injections. It may take some more time for recovery with Oral tablets.
In my opinion, you should consult a Surgeon get examined once again. I have seen cases with infection extending upto the abdominal wall.
As of now, your infection is confined to the back. Sepsis is prevented by Intravenous Antibiotics. Low grade fever is part of the entire treatment process even on antibiotics. It will come down.

Hope I answered your query. If you have any questions, I will clarify further.
Wish you good health
Regards
