Suggest Treatment For Oily Skin And Acne On Forehead

Question: Hello There,

I am XXXXX, 26 years old. I have a oily skin & got pigmentation marks forehead, what do you suggest me to get rid of the oily skin & the marks, i also have bumps on my nose, this looks ugly. I use fash wash stopped using soap.

I am Dr. Kakkar. I have gone through your concern and I have understood it.

Acne and oily skin (seborrhea) are interlinked. Pigmentation over forehead, I guess is due to post-inflammatory hyperpigmentation which is a common sequelae of acne.

If I was the treating doctor I would have started you on Oral retinoids (Isotretinoin). Isotretinoin is sebostatic (would decrease the size of sebaceous glands to <90% within a month and would thereby reduce oiliness) as well as anti-acne. Oral retinoids take about a month time to show visible results. They need to be continued for at least 4-6 months. Retinoids are taken under the supervision of a dermatologist because they require lab monitoring e,g Serum Lipid profile and Liver Function Tests.

Presently, you could begin using a 2% salicylic face wash/foam (saslic DS face wash) and once the retinoids have there full effect in about a months time, you need to stop salicylic acid face wash (OR else they both together would overdry your skin) and begin using a gentle cleanser for face wash.

Regarding the bumps on your nose, you could use a topical antiacne medication, locally. My choice would have been benzoyl peroxide 2.5% gel twice daily.

An Oral antibiotic e.g Azithromycin may also be required if the bumps on your nose are inflammatory/pustular. Azithromycin can be continued for 4-6 weeks depending on the response.

Regarding pigmentation over forehead, you could use a cream containing 2% kojic acid (Kojic cream), twice daily, over the spots for a few weeks. Kojic acid is a skin lightening agent and works well for post acne hyperpigmentation.
