Suggest Treatment For Night Sweats And Weight Loss

Question: Hi I've been unwell for 7 yrs with suspected kidney infections and 2 positive urine test and 2 normal for carcinoid. I am boiling hot all the time sweating but with low body temp. My white cell count is 15 - 25 for many yrs. Scans only show small lesion on liver they think is a birth defect. Anti biotics work for a week but all starts again. Night sweats, constant pain in back and bowels. Pain in kidneys and right side below liver but no gall stones. Diarrhea, weight gain and weight loss. The constant burning heat in my brain unbearable feel like giving up or breaking down. Please do u have any suggestions no one is helping me even tho seeing various specialists.

Dear Sir, I can provide u with a detailed account of the past 7 yrs (if I have the space) unfortunately I have no medical notes to scan as I am from the UK and it takes weeks to get them. I have a history of irritable bowel syndrome. I have been diagnosed with gastritis and oesophagitis after endoscopy and now take lansoprazole. Had 1st kidney infection 2003. Suffered right-sided weakness after traumatic brain injury in 2004 (regained movement but not quite the same ever since). Had microwave endometrial ablation and tubular sterilisation - due to extremely painful and heavy menses 2005 (already had children - 3 premature but natural births). After this another kidney infection. From 2006 chronic kidney infections. Chronic diarrhea. Sickness. Dizziness. Headaches. Low immunity to bugs. Very hot and sweating. Heart palpitations (take beta blockers). 1st xray showed small abnormality on kidney so under anaesthetic put a camera up my urethra to right kidney. Kidney structures normal and no cancer. Unable to control pain when awoke. At home had 2 days of dark red urine and pain. Woke in middle of the night in worst pain ever screaming. Taken to casualty by ambulance. Purple urine with clots. Severe kidney infection and not functioning. Took 4 types of iv anti biotic to overcome severe infection. In hospital for 8 days. Felt well for 3 months but infection started again. White cell count never dropped below 15 so haematology requested to examine blood looking for lymphoma and leukaemia. Findings were: blood responding normally to bacterial infection. Kidneys suffering constant reoccurring infections ever since. Last year blood resent again to haematology as wbc raising to 25 and back to 15 after each infection. Still blood responding to bacteria. Many ultrasound - no kidney stones, no gall stones, no tumours. Ct scan showed small lesion on blood vessel at front right of liver like a tube. Mri scan. Results were benign lesion. Did 5haii 24 urine test on special diet results 69. Vomited blood and taken to hospital by ambulance blood pressure 50/70. Endoscopy showed nothing. In hospital 7 days blood pressure high then low. Octreoscan arranged. Results normal. Another 5haii urine test. Results 59. Special blood test for carcinoid activity. Results normal. 2 further urine tests for carcinoid results normal. My problem is this: My kidney infections have become worse and more often. Urology don't believe infection is in kidneys but everyone else thinks they are! Takes 6 weeks of symptoms before painful urination. Urine samples have blood and white cells but no bacteria. Urology are certain there is nothing wrong with my kidneys - they believe kidneys are picking up the infection from my blood. I have constant pain in my right side below ribs. My wbc reaches 25 showing severe infection. Constant bad back and rib pain. Swollen glands that move and go up and down. Headaches. Very hot and sweaty. Cold sweats. Swollen stomach. Swollen ankles. Fluctuating blood pressure. Fluctuating weight can gain 2 stone lose 2 stone. Severe night sweats. Endocrinologist unsure if I have carcinoid as tumour/s don't show on scan. Hepatology certain lesion benign unless carcinoid. Diagnosed with carcinoid syndrome and then told its not carcinoid syndrome! No one knows where infection coming from for sure. My life is very small. Given up my job as too unwell. Frequent urination day and night and infection symptoms have stopped me travelling. No stamina. Use denial as coping strategy and keep fighting it. But warn out, getting depressed. No answers or firm diagnosis. Referred to gynae again to look for endometriosis..? Then referred again to urology after that! Haven't seen these yet but feels like a large circle!! I'm suffering, ill and in pain and too hot and worried about infection killing me while they look for carcinoid. Also I don't like to take anti biotic every couple of months. Suffered severe allergy to nitrofurantion anti biotic 4 yrs ago which was horrific. Effects of anti biotic are now bad. I need some answers and treatment but no one will commit to diagnosis or treatment. 7 yrs. It's ruined my life. I have 3 children and a widowed mum. One of my children is aspergers and needs care. I need to be well. Please help me.

Apologies for late reply.
White blood cells constantly elevated, in the presence of normal blood picture by peripheral smear one would definitely suspect the possibility of infection as cause of your symptoms. As you had documented urinary symptoms like red urine , blood clotts following Cystoscopy, the cause would definitely would have been attributed to Kidney as source of infection.

If the following criteria are fulfilled then kidney is unlukely to be source or cause of infection
1. Normal Urine Microscopic report
2. Normal ultrasound / ct scan: normal Coritomedullary differentiation, no subcapsular collection, absence of Hydronephrosis and Hydrourter( enlargement of kidney and ureter respectively).
3. Absence of bladder wall thickening
4. Absence of Vesicoureteric reflux

Going by your description, i assume all these criteria are fulffilled. Kidney is unlikely to be source of infection. But even then i would like to rule outr chronic infection like Tuberculosis as cause.
Tuberculosis may be rare in UK, but still likely in view of symptom that you are experiencing.

Had you been my patient, I would have further proceeded by these steps

1. Bone marrow examination
2. urine examination for Tuberculosis

I would suggest you to discuss it with your treating Physician.
